Suggest Treatment For Fever And Stomach Infection
Question: Hello my son who is 13.5 months old was having high fever 101 from 15th May, so the doctor prescribed him Ofloxacin Suspension and said that he has stomach infection. But he was still having fever after three day course. So on 18th the doctor admitted him and did a blood and urine test. The results are: TLC was 19.25 and in urine test Pus Cells were 5-7 WBC/HPF. So the doctor started from 18th the following injections: Fortum 500 and Tazar 1.125. Medicines: Z&D DS 20 and Enterogermina. He was discharged on 19th the next day, but the doctor told to continue these injections and medicine for another three days that is till 22, but now the doctor has said to take it another 2 days the injection and medicine.
So I am really confused what’s going on. He doesn’t have fever now and he is perfectly active. So is it okay to take these medicines and injection for seven days. He is going through lot of pain because of these injections so I wanted the suggestion that is 5 days enough that is till today 22nd or should I give him injections and medicine for another two days? Please advice.

I would like to continue the injections for another two days.
The reasons are
1)The child's age is of 13.5 months only and the babies of this age are prone to develop severe sepsis if the dose or duration of medication is not appropriate
2)Although the report showed increased leucocyte count and pus cells in urine, both suggestive of infection but diagnosis is not certain as more specific markers of infection like C reactive protein and culture sensitivity of urine not done.So it is difficult to assess the severity of infection
3)Baby had fever even after 3 days of ofloxacin.This may suggest that the infection was severe and in such condition treatment should be given for appropriate duration.
4)I can understand that intravenous injections are very painful,but as child's fever has responded and general condition improved with the medication it should be given at least for 7 days.The recommended duration of antibiotic is 7-14 days depending on the severity.The inappropriate duration may cause recurrence of infection and also can cause resistance to that particular group of antibiotic.
So in summary I would like to give it for another two days(even if child is active and playful) to be on safer side.

Hello sir,
Today the doctor told us to stop the injections because his canula came off. The doctor has given oral medicine that is cefspan and enterogermina and Zinc DS. Please suggest the treatment been given from the first day is it correct?
Also, he is 13.5 months but his weight was only 8 kg before the treatment. He doesnt even walk also, he just stands on his own but doesnt walk ahead. I am really worried about his development, please recommend some medicines.

The cefspan is a cephalosporin group of antibiotic.Initially he was on tazar and fortum.Fortum is also a cephalosporin group of antibiotic,so shifting to cefspan is correct.Continue Zinc syrup for total 15 days.You may need to give cefspan and enterogermina for 4-5 days.
Weight of 8 kg at 13.5 month is low.If at this age he is standing without support this is normal.I would be worried if he won't be able to walk till 18 months of age.So no problem regarding development.
Give him adequate nutrition,feeds rich in calorie.You can make feed calorie dense by adding ghee/butter to it.Decrease the frequency of breast feed and milk intake increase the volume and frequency of standard meal.At least 3 standard meal with two to three snacks and night time breast feed.
