Question: Hi , my 5years child was suffering from fever and dehydration in the summer. He is frequently urinating and very little. Early morning at 8 am we gave him fever and stomach ache medicine. We had a urine test at 11 am and found ketones positive. Dr said nothing to worry and asked to have water frequently? Does it indicate diabetes in the kids?

I have gone through your query carefully and understand your concerns. Ketone bodies are frequently found in the urine of children who do not feed well. Ketone bodies indicate a state of starvation and can occur if there is prolonged gap between meals.

You have to ensure that he takes at least small amounts of food at regular intervals. Plenty of fluid intakes (including ORS) is also advisable.

In any case, it does not indicate diabetes.

Let me ask you about the problems he is facing with his foreskin. If the foreskin is retractable and unless there are frequent infections (redness, pus formation etc.) I do not suppose that circumcision needs to be done.

No redness or puss formation etc. Recently he started saying burning sensation or heat while passing urine or after. When he was 3 yrs DR gave ointment and advised to apply on the foreskin and massage which we did and left it. When we went to consultation today for fever and stomach / urine burning sensation he checked and mentioned about circumcision.

I do not suppose that a mere burning sensation would indicate balanoposthitis or the infection that I was referring to. I would insist on manual too and fro retraction and replacement of the foreskin. If that can be done with ease I do not suppose circumcision is needed.

I understand that your doctor is assuming the fever to be an effect of a local infection and has suggested circumcision but clinical correlation is needed. I would insist you to tell me if the foreskin is freely retractable.
