Brief Answer:
Will answer one by one regarding diet and medications.

Detailed Answer:
Hi,

I have gone through all the reports that you have mentioned here.

SGPT is slightly elevated which here could be due to grade-II fatty liver.

This value has no clinical significance in an otherwise healthy individual. I would not be worried about it.

Vitamin D (6.53) is significantly low here,so needs external correction.

HDL cholesterol is also low and rest of the reports are almost normal.

(Total bilirubin normal value: 0.3 to 1.9 mg/dL,triglyceride normal is up to 170 mg/dl and serum globulin: 2.0 to 3.5 g/dL)

A) Here, I would like to say that fatty liver is not alarming finding. It indicates fatty tissue deposition on the liver. Therefore I wouldn't be personally worried about this finding.

Generally, fatty liver condition is a kind of obesity. And grade-ii fatty liver is completely reversible in the initial stages specially, by following below mentioned corrective measures.

1) do regular exercise or brisk walk at least 30 minutes per day

2) Cessation of alcohol intake (if you are alcoholic)

3) Avoid junk foods and fast foods,

4) Also take low fat and high protein diet (Veg- Pulses, soyabean and cheese)

5) You should take more fresh fruits and vegetables.

Although there is no such a time limit on when to have a repeat ultrasound scan but it can be repeated after 3 months following above corrective measures to check the liver status along with some liver function tests (SGPT,SGOT and Billirubin).

B) You can take vitamin D3 cholecalciferol in sachets forms 5000 IU per day or 60,000 IU once a week for at least 8 to 10 weeks.

The major cause of vitamin D deficiency is lack of proper sun exposure or remains covered throughout the day.

Apart from treatment, you should have adequate sun light exposure preferably in morning hours around 8 AM.

If absorption or compliance is poor then one can give vitamin D3 bolus by the help of physician.

C) The main reason for HDL to be low could be lack of enough muscular exercise.

Initial 20 minutes of walk goes in burning your sugar and then the fat start burning which should happen for another 20 to 25 minutes through a stable, moderate speed of 5 to 6 km/hour and then you will certainly see improvement in the HDL cholesterol.

In general,to increase HDL and (to reduce Chol/HDL ratio) follow these guidelines:

- Avoid direct sugars,
- Avoid fried snacks, bakery items,
- Have ample of vegetable salads,
- Snacks should contain healthy nuts like almonds or walnuts and a few dry fruits but no fruit juices
- Avoid soft drinks or hard drinks and smoking cessation.
- Simple Chapati and steamed rice or rotis made up of different whole grains should be your staple diet.
-vitamin B3 (niacin) increases HDL cholesterol

Hope this information will help you.
