Brief Answer:
It is in his mind

Detailed Answer:
Hello,
Thanks for using Healthcaremagic.

I read your query and understand your concerns about the problems faced by your boyfriend.

However I do not understand why surgery was given as treatment option. I do not think any other other condition except varicocele responds to surgery when we consider erectile dysfunction.

Since he is able to have normal erections while having phone sex and all his blood investigations are normal there is high possibility that he suffers with psychological erectile dysfunction.

The condition is best treated by counselling and better understanding of the problem. I understand that you do not want to go to doctor but unfortunately a doctor having in person consultation can offer counselling and it is almost impossible to provide counselling in question answer form.

I also like to assure you that there is little role of medication in psychological ED and he is not required to take any medication.

I hope this answers you.

Brief Answer:
I think YES

Detailed Answer:
Hello,
Thanks for follow up.

Delayed ejaculation or retarted ejaculation most often results from stress arising from various factors. I have observed in my practice that experience with phone sex is quite different than the intimate encounter with loving partner.

I understand that he is stubborn and emotional to a great extent but does not mean that he can not ejaculate. While he comes to you try to make him comfortable and do not allow to think over too much about his sexual problem.

In my opinion his emotional nature and depressed feeling is most likely cause for his delayed or absent ejaculation.

I also think that having blow job and other forms of oral sex should be preferred during initial sexual encounters rather than penetrative sex. This is important for him to gain confidence and to achieve the level which he used to have few months back.

I hope this answers you further.
Thanks and regards.
