Question: I am a 30 Yr Old married guy, Its been 2.5 yrs since my marriage. I have been able to do intercourse only a couple of times as my wife was scared to do it for almost 2 years. Off late i am not able to get good erection and if i get am not able to maintain it for penetrating. I get very nervous before sex and loose interest.
I badly need some help &medication in having better and longer erection and also want to cool my nerves. I am lacking confidence now.

First regarding the testosterone level, I like to tell you that it is associated with lack of desire rather than erectile dysfunction. Since you have normal level of testosterone, it is expected to have the normal desire and I assume there is no problem in sexual desire.

Erectile dysfunction can be due to multiple factors and psychological factors are the one of the important cause of this. It is diagnosed after considering all relevant factors and which means there is no abnormality in the system but the excessive anxiety makes the erection difficult or impossible.
Generally this is the most common cause of ED worldwide and fortunately it is treatable.

Before we proceed further I have few questions for you
1 How long there is problem?
2. What exactly happens when you engage in foreplay and think about erection?
3. Do you get your penis erected while you come out of the bed or in early morning
4.Do you have erection while watching porn
5. Do you masturbate, if yes frequency per week
6. Do you have sexual experience with different partner in recent years ?
7. If yes, how was the experience in compare to your wife
8. What is the frequency of sex and successful sex per week

Dear Dr XXXXXXX

Please find my response to the questions raised by you.
1.Its there from past 1 year.
2. During foreplay I get erection. However when trying to penetrate its goes off.
3. Yes in the morning.
4. I do have erection while watching porn.

8. I just had 2 successful attempts in past 1 year. Usually am worried about being unsuccessful and hence try to avoid it.
If its psychological please let me know some medicine and other ways to tackle it and regain my confidence. Also something which can give longer erection and increased desire. My desire is dying down due to lack of confidence.
Are there any medicines to reduce my anxiety before sex?

After going through your query it is clear that the problem is completely psychological one need to manage with psychological means. I like to assure you that it is completely treatable.

As most of the treatment of ED (Erectile dysfunction) is done with talk therapy it requires a lot of understanding between patient, his partner and a doctor.
I like to tell you that now you can stop your current medication and contact a psychiatrist in local area, who will explain you methods to control stress and and fear which is the cause of problem.

I like to emphasize again that Medicine is the secondary form of treatment and need not to be successful.
