Question: Hi,
I am married for 8 years now, have a child aged 4.
I am in intercaste (love ) marriage, all seemed ok, but since last couple of years, I feel separated from my wife, I don't feel the emotional/physical bonding with her, I don't know the reason why?
Though she is very caring and supportive but i have been very irritate have not been behaving well with her,.
We do not have sex for months at large, its not that I am interested in any one else or something like that, but either we don't get chance(as we are in joint family) or too exhausted (both working).
I read a lot about sex in internet and also could make out that, I have a problem of pre mature ejaculation, i cannot sustain for a long time maximum(2-3 mins), i sometime feel guilty also of the same.
My marriage is not at risk by any chance, but I feel low and the spark that was their during my love days are missing and I feel guilty of not being able to perform in the bed.
I was a smoker and off late have quit it.
Please help!

You are facing a typical urban life style problem.You both seem to be under a lot of stress , always running after targets or trying to get promotion, so sex finds a lower place in the list of priorities. It is a general trend I see daily in my practice. Stress causes lesser interest, you can't focus on the pleasure so penis gets soft, You have ED , producing anxiety which further worsens the previous problem. It becomes a vicious circle. You need to sleep for 7 hours, 30 minutes morning walk, balanced diet and stay away from any drug.
you should get Your Serum Testosterone (Free), HbcA1 and fasting blood sugar, serum prolactin done and revert back.

Dear Sir,

Thank you very much for your reply and appreciate your kind gesture.

As far as working is concerned we have been working from the day 1 we met and even during our days of courtship, even after a whole days work, I used to fly down to XXXXXXX for week ends as I was posted in XXXXXXX to meet her and spend the weekend together, at that time we use to face monetary problems also but we never bothered and lived life king size, after the birth of my child, or rather during the pregnancy period and after that when my child was aged 1 year, I remember we never touched each other, and since then the urge has vanished.
As per the test is concerned, I don't what is the test for "Serum Testosterone (Free)", but the lab people were very skeptical and were looking at me in a different version altogether, though they have taken the test, they want a prescription to process further with my history, now what should I do.
May I request you to kindly issue me a prescription with the history as discussed with you and the mention the test to be done, also they said the reports will take 7 days.
I need and request your help!

These tests are done very routinely and there is nothing to glare at . You should choose a very good lab like Ranbaxy, Thyrocare or Dr Lal . Yes, they take 3-4 days for the result. Sending prescription on line is not possible. I read in another query posted by you where you had considerable depression some time back, If this is so, present problem could be related to depression. If they will give report after 7 days, wait and don't worry about their looks.

Dear Sir,

Thank you very much for your suggestion.

I went to a psychiatrist also and explained him my position and he has prescribed me the following medicines.

zicam 0.25 X 3 (2 times, afternoon and night) and feliz-s (escitalopram) (half tablet for 4 days in the morning and than 1 tab each daily for a month) and then follow up.

Do i start paroxetine 12.5 mg also.

if you have already started Feliz -S, continue first with 10 mg and after 3 weeks, if no improvement, increase to 20 mg. I wouldn't start paroxetine yet.
