Brief Answer:
Splenomegaly & cystic kidney

Detailed Answer:
Hi,

Thank you for your query. I can understand your concerns.

Cystic kidney found on ultra sound test:

The distal renal tubules develop several diverticula over the course of aging. These diverticuli may be precursors of simple cysts, which are often found in the aging kidney.

Enlarged spleen is most likely due to extra-medullary hematopoiesis as chronic kidney disease on hemodialysis is often anemic.

These are the possible explanation in a patient with chronic renal failure.

However there are many other causes of splenomegaly for which detail history and physical examination is mandatory.
