Suggest Treatment For Elevated Creatinine Levels

Question: I had just lab work on August 26th. My Creatinine was1.34, BUN 16 and GFR 53. On April 23 of this year my Creatinine was 1.06, BUN 16 and GFR >60. On November 17th of last year my Creatinine was .91, BUN 21 and GFR >60. It appears my Kidney function needs addressed. These are fasting numbers. Please advise.

I understand that your creatinine has significantly increased from last year. There are many reasons why this can occur, the most common being dehydration.
In order to help you in a better way I would like to know a few more details

1. Was any other tests like urine routine and ultrasound of the kidneys done ? please furnish those reports
2. Please mention all the tablets you are currently on right now , some drugs can increase creat
3. do you suffer from any co morbid conditions ? like diabetes or hypertension ?
4. do you have any difficulty in voiding urine? do you pass urine more frequently than before?

Dr XXXXXXX I have had many imaging tests, MRI, Hida scan, CT scan, Ultra sound and x rays, and have seen many doctors in the last 18 months because of stomach issues. My diagnosis is IBS. I do have benign cysts in both my pancreas and kidneys. My last MRI was late January. I had a complete microscopic urine test 15 months ago and all my values were normal. My regiment of medications include 25mg Zoloft and 10mg Pamelor. I take vitamin B6 and B12, vitamin D, Omega3, a Probiotic and melatonin at bedtime as supplements. I do not have Diabetes, my glucose was 104, but 2 days ago it was 98. My blood pressure has always been normal, no Hypertension. I have no problem while urinating and my frequency has not changed.

Thanks for getting back with the details.

Your serum creatinine is slightly on the higher side and there is no obvious reason for it. The drugs you are taking is not nephrotoxic either. This is how you should go about it

1. The most likely cause for the increased creat in because of your hydration status. I recommend that you get your creat checked once again after drinking plenty of water
2. I suggest you to repeat your urine routine and see if there is any ongoing infection as well. Treatment with antibiotics will help settle the infection
3. Another possible reason is prostate enlargement with retention of urine, ultrasound of the pelvis with post void residue must be done to rule this out

Dr XXXXXXX your response is very explicit. I do feel the dehydration is most likely. I do work out and I use a Sauna. Also, I had only a small glass of water 12 hours before the test. My Prostate has been examined by a Neurologist and I have some enlargement but all is normal. Thank you and I will close out my query. I will give you an excellent rating, but when I do the rating,the box has no place to submit.

Hello

A lot of research is currently going on how to improve kidney function, but most of them have ended up with drugs and lifestyle changes which prevent progression of the kidney disease. There is no way to improve the kidney function as of now but the following are the ways to prevent progression of kidneys damage

1. Excercise daily for a minimum of 30 min. Especially cardiovascular exercises
2. Avoid nephrotoxic agents like herbal medicines and pain killers
3. avoid dehydration, keep yourself well hydrated with water
4. prevent recurrent urinary tract infection especially in high risk individuals like diabetics, anatomical abnormalities in the urinary tract, etc
5. follow a low salt diet
6. antioxidants like vitamin c and e have shown some help
