Suggest Treatment For Dry, Itchy Patches Of Skin On Elbows And Knees

Question: I have very dry, very itchy patches of skin on my elbows, knees, and two knuckles. The patches initially began with a few white flaky spots and itchiness then grew outward to be know peeling and cracking and severely dry. The patches on my knees and elbows have been there for about 2 months now and the patches on my knuckles have only appeared in the last week or two. I have tried lotion twice a day with no change and I have tried hydrocortisone twice a day with only slight improvements on my elbows. Any advice?
