Suggest Treatment For Dry Cough

Question: Hello Doctor

My mother is 65 yrs old. She been suffering from acute allergy which is causing dry cough to here. Cough happens after every 5-6 seconds and she is not able to sleep during night.
We have seen a local doctor and the treatment she had prescribed to her is as follows:

18.9.2015
Histafree
Grinlintus
Zevit

it went for 30 days and it a relief to her a lot.

More recently (In October 2015) she again started with cough. and the medicine prescribed are as below:

Tablet Zindeen
Vprine D3 1K

She was asked to take it until 29th Nov.

Today (30.11.2015) She has been prescribed as below:

Tablet Deriphillin 1BD
Tablet Ebast 20
Tablet Azee 500

The above medicines are to be taken for 3 days (30.11.2015 - 2.12.2015)
Her blood report is normal as per the doctor. She says allergy has caused enlarging of internals cells and her throat is clear. She is unable to find as to which thing she is allergic too. We have been asked to take this medicine for 3 days to get relief. If it does not then she will recommend to a specialist.

I have written all these to you as to what is to be done, Is the on the right course. If you want I cant send the blood report to you. I think there is a call faciliy under the subscription for my company. Do you need to speak to her before prescribing anything. She finds it difficult to sleep during night and does cough most of the time. This situation arises after every some days of relief.

I can understand your concern.
By your history and description, possibility of allergic bronchitis is more in her case.
And for this, inhaled bronchodilators and inhaled corticosteroid (ICS) are the mainstay of treatment.
Till date she was not improved fully with oral drugs.
So in my opinion, you should consult pulmonologist and get done
1. Clinical examination of respiratory system
2. PFT (Pulmonary Function Test).
PFT is must for the diagnosis of bronchitis. It will also tell you about severity of the disease and treatment of bronchitis is based on severity only.
She will need inhaled bronchodilators and inhaled corticosteroid (ICS).
Oral combination of antihistamine and anti allergic drug is also beneficial in her case.

Hello Doctor

Find attached her PFT and Blood test report. She is currently on the below medications:

Tablet Deriphillin 1BD
Tablet Ebast 20
Tablet Azee 500

She will take these until tomorrow morning. Please suggest what should we do now?

Thanks for your follow up question on HCM.
I have gone through the reports you have attached.
Her PFT is normal. So no need to worry for asthma or allergic bronchitis.
Inhaled treatment is given if patient is having abnormal PFT. So she will not need inhaled bronchodilators and corticosteroid.
In my opinion, you should continue antihistamine and anti allergic drug combination in her case after completion of current medications.
This combination is easily available in the market. It should be given at night time before sleep. She should take it for 1 month .

Thanks Doctor. I went to see a near by ENT Specialist in a hospital. He told us the same thing. Even her X-ray report ruled out Pneumonia.

He has given her 7 days of medicine course to be taken which is mentioned below:

Tab. Augmentin 625mg (5 days)
The below for 7 days:
Tab. Pan D
Capsules Vizylov
Steam
Tab. Musinac
Grilinctus cough syrup
LCZ


Hope she recovers after this as this is due to allergy caused by may be Pollution. Causing severe respiratory ailment (dry cough, sometimes wet) accompanied by nothing.
