Brief Answer:
Get done PFT and sputum examination for AFB.

Detailed Answer:
Hello dear, thanks for your question on HCM.
I can understand your situation and problem.
You are having chronic cough , breathlessness with weight loss. So following conditions should be rule out first.
1. Tuberculosis
2. Bronchitis
3. Lung cancer.
Since your chest x ray and CT show fibrosis, lung cancer is less likely.
Possibility of bronchitis and tuberculosis are high.
So better to consult pulmonologist and get done
1. PFT ( pulmonary function test ).
2. Sputum examination for AFB.
PFT is needed to rule out bronchitis.
Sputum fir AFB is needed to rule out tuberculosis.
Also get done TB PCR test from lymphnode biopsy.
Hope I have solved your query.
I will be happy to help you further.
Wish you good health.
Thanks.
