Question: Hello Doctor,
Pleasant day to you,
A street dog bitten my thighs with my thick jeans on when i was riding the bike. I had a small bleeding but my jeans remained intact there was no holes on my tough jeans. Logically speaking the saliva from the dog has not been transmitted through my thick jeans but due to friction i have small wound, Instantly i cleaned the small wound with savlon antiseptic. Its been 48 hours now i have already got a TT shot, Should i still get vaccinated for Rabies or not. I really do not want to get series shots of rabies vaccination I am worried. If i am not vaccinated for rabies Kindly advice me what is the risk level, high level risk, low level risk or no risk.

Brief Answer:
Please take anti rabies 5 dose vaccination schedule

Detailed Answer:
Hi xxxxxxxxxxxxxxxxxxxxxx,
Thanks for writing in to us.

I have read through your query in detail.
Please find my observations below.

1. Your bite is a high risk category because it involves stray dog and there is bleeding.

2. Vaccination is important because there was bleeding and in dog bite cases the vaccine is recommended for 5 dose schedule on days 0, 3, 7, 14 and 30 starting from today. This is international recommendation.

3. Please consult your doctor and start vaccination today.

Hope your query is answered.
Please do write back if you have any doubts.

Hello XXXXXXX

Thank u so much for your prompt response. But the bleeding was very minimal and i observed there was no single torn in the jeans and No saliva existed over the jeans. Do you still recommend to take 5 dosage. How about taking a single rabies shot will that be effective or give me another alternative of consuming tablets or a single powerful rabies shot.

Cheers,
Praveen

Hi Doctor XXXXXXX

This would be my final question to you.

As per your advice i have taken a Rabies shot today. The Nurse in the Hospital Injected to my Buttocks, I have heard age old practice of injecting the vaccine over the wound area or over the Bowel/ stomach area. Can a vaccine be injected any part of Human body. Please enlighten me.

I am still thinking of getting 3 shots of rabies instead of 5, will the 3 shots be effective or not. Appreciate your time and patience.

Cheers,
Praveen

Brief Answer:
Please take anti rabies 5 dose vaccination schedule in your arm

Detailed Answer:
Hi,
Thanks for writing back with an update.

1. The anti rabies vaccine like rabipur injection should have been given in your deltoid muscle in the arm. Probably the nurse is not well informed and that is why she gave in the buttocks. Next time please take it in the deltoid muscle in the arm.

2. The old practice of giving injection over the abdomen is stopped 15 years back and no longer used because it caused reactions. This vaccine is to be taken in the arm and is safe.

3. There is immunoglublin injection which is given in the wound when it is deep. In those patients immunoglobulin is given on day 0 in the wound or buttocks and anti rabies vaccine like rabipur is given as 0, 3, 7, 14, 30 dose in the arm

4. The idea is to build your immunity by taking vaccine. Todays vaccine is given in buttocks and therefore not in the right place and will not work well. Therefore 5 doses are required to give you immunity and will be effective.

5. I want you to take precaution now so that you will not suffer later. Therefore please take the 5 vaccine dose recommended internationally.

Hope your query is answered.
Please do write back if you have nay doubts.
