Suggest Treatment For Discomfort Testicles

Question: My husband has testicular discomfort ...sudden onset 3 days ago....only at night once he takes off his underwear and puts on lounge pants. There is no swelling, no lumps, no urinary symptoms , no fever, and no sexual issues. During the day he is fine and is not bothered by anything. Normal BP and only medication is Paxil 20mg for claustrophobia/ restless legs issues, which helps. He is 64 yrs. old and in good health.

Thanks for your query and an elucidate history.

In the absence of any other related symptoms you noted, I would think of the following :
Varicocele meaning dilatation of the venous plexus at the cord and testes.
Once the underwear is taken the scrotal support may be gone and in lounge pants there can not be any support and then the discomfort would come.

I would advise you the following:
Please ask him to see if there is any swelling around the testes ?
Get an ultrasonography and color doppler examination of the scrotum and the abdomen particularly for the genito-urinary system to find any hidden / occult problem, like a mass in the testis / prostatitis or enlargement of any of the organs including the testes / prostate and so on.
To get the blood and urine tested.
Maintain the testicular support even after he changes over to the lounge pants.
Take Advil only if required and take Ranitidine or PPI to avoid gastritis associated

Urologist's opinion is always better to be taken as a direct clinical / physical examination by a Surgeon ( Urologist) can elicit some signs that the ultrasonography and color doppler can easily miss.
To continue Paxil.

My husband felt the symptoms were quite similar to a previous prostatis infection, and at that time he was treated with Septra 800mg bid x 14 days with resolution and no adverse effects.We carry this medication with us, just in case it flares up again, as it has worked in the past. ( We are Canadian snowbirds and as such have only emergency medical coverage ) and so he started Septra with a first dose Dec.7. After the third dose, he developed a few hives, took 50mg Benadryl with relief. He has had the same reaction from Penicillin, and Ciprofloxacin ( hives ) and now Septra. What other antibiotic could he take for this, dosage and duration.
He felt his symptoms were improving even after 3 doses of Septra.

Septra 800 (trimethoprim and sulfamethoxazole) causing hives in the second course is very rare, but yes, chances should not be taken. These medicines are not related to the Penicillin or Ciprofloxacin group.
Well it is better to stop.

It is good that Benedryl worked well.

You can try Cefixime 200, I am afraid for this is a prescription medicines and you need a prescription of a Doctor. He can take this for 14 days.

Cefixime 200mg ...how many times a day or just one dose daily for 14 days?
He did insist on taking Septra 800mg again this morning, about 1100am ( 4th dose ) and as such has had no reaction as of yet...I am watching closely and have Benadryl on hand.

The Dictum of allergy is : ''anything under the sun can cause allergy'', meaning anything in the world can cause hives and since He has already taken for 14 days in past, the likelihood of it causing hives is rarely known. Sulpha commonly causes fixed-drug-reaction in which one gets a black spot on the skin which itches and comes back again if one repeats the medicines.

Well, Hope to see no reaction and anyways benedryl is ready. You may also have steroids with your Use if there is recurrence of allergic reaction.

Cefixine is given in twice daily dosage under the prescription of a Doctor , can be given twice daily.

I hope to see no further reaction.
