Question: Hello Doctor,
My 2.9Years old son is not passing the urine properly, he passes 3-4times for 24hours time. 700-1000Ml liquid intake in the form of water/watermelon/tender coconut/milk. While he passing, the flow of urine and amount looks good. But sometimes he just stop the urine by pressing and he is not saying any complaint in pain. No infection noted from culture test. But worrying about how much he should pass, how many times? The problem is for past 10 days and today only he is having fever. Today he passes twice from the mrng.

Brief Answer:
Looks like strangury....common in summer, don't worry

Detailed Answer:
Hi.... I understand your concern. This looks like strangury, which is very common in summer. This occurs when the fluid intake is less than appropriate. I suggest giving him one full glass of sugar water with 4 teaspoons of sugar in it, followed by 2 or 3 glasses of normal water. This will settle the issue. To prevent this you need to give at least one and half litre to 2 litres of water per day.

Another issue is that I am not able to download the reports uploaded by you.

Kindly upload once more. If there is no fever and vomiting, it is highly unlikely that this is urinary tract infection.

Thank you doctor.

One more thing, his pennis external layer is not moving back and it is almost close to the front. does it common in his age? do we need to perform surgery? is there any treatment apart from surgery?
I have reuploaded the reports.

Brief Answer:
Steroid cream if applied at prepuce may avert surgery

Detailed Answer:
hi..... greetings from XXXXXXX I am a consultant Pediatrician at XXXXXXX Kamakoti CHILDS trust hospital in XXXXXXX

The tight prepuce is common in this age...but if it's too tight it requires either steroid cream application and if not better with this treatment, circumcision may be required.

Tight prepuce which is requiring circumcision may get better (need for surgery might be averted) if steroid cream is applied at the prepuce in appropriate technique. This needs to be taught to you by your doctor.

I suggest you get back to your Pediatrician regarding this.

Regards
Dr. Sumanth
