Suggest Treatment For Diarrhea In Infant

Question: Hello Dr.

My 2nd baby [out of twins] is suffering from loose motions.
Current frequency of motions is around 5 times in a day and it has been started since day before yesterday [last Saturday] and only on 1st days he had little redish in his stool but since yesterday no reddish stool and only somewhat sticky and somtimes quantity is less.

Most importatnt is this is 3rd time he is started with loose motions sice last 2 months. so please let us know why loose motions comes after 2-3 weeks and is there any sever in this?

Also please suggest the medication on it.. Current medications are started as below:
Enterogermina
Z & D
and WHO ORS water.

Crurrent Age: 8 months and 9 days

May be cow's milk protein allergy

Detailed Answer:
Hi...Greetings from XXXXXXX After going through your question in detail - this is my opinion and this is what I would have done if I were your treating pediatrician.
Possibility -
1. Cow's milk protein allergy is a strong possibility as there as blood in the stool in exclusively breast fed baby.
Suggestions -
1. Mother should go off the cow’s milk products completely - including biscuits and chocolates and all products related to milk.
2. Continue exclusive breast feeding and if it is not sufficient you can opt for Zerolac.
3. Cow's milk protein intolerance in transient as it will go off as the kid grows up.
4. Avoid all cow's milk products till the kid is 1 year age.
5. Mother's milk is the best for good growth of the baby. Rather than stopping mother's milk, it is best for the mother to avoid all milk products and continue breast feeding.

Hello Dr.
Thanks for the information!

We had started COW milk just 1 month and 15 days back.
AS per your suggestion we will stop the COW milk.
Also we are giving below feeds to our babies:

1. CERELAC stage 2 [wheat apple, whaet rice, wheat cherry or honey, Wheat orange]
2. Rice dal paste [kind of in lisemi liquid form]
3. XXXXXXX
4. RAVA kheer

Please suggest along with these feed if we need to continue on any other feeds


Also please suggest that what to do with another baby [from twins who is not having loose motions] and that baby takes good amount od COW milk feed andmostly we have seen that he liks much COW milk.

Should we continue on the COW MILK for 1st baby or not?

You can continue Cow's milk for 1st baby

Detailed Answer:
Hi...as the 1st baby has not developed any cow's milk allergy, you can continue the cow's milk for that baby.

But be on the look out for this symptoms in the 1st baby too.

Complimentary feeding -

At this age you can start giving Cerelac stage one rice or Nestum rice stage 1 etc. All are same in calories and energy. Only thing is palatability which is subjective for each kid.
Other home-made food options will be -
1. Daal and rice well cooked and made into a porridge along with ghee
2. Boiled apple or banana and mashed into a paste.
3. Any made cereal porridge preparation with additional ghee added.

Remember certain principles like -
1. Add one food material / cereal or fruit per week. This will be useful because if he develops vomiting or diarrhoea, we will know what to avoid.
2. Add vegetables and fruits made into a paste surely to avoid constipation.
3. Do not add too much sugar as it may cause diarrhoea.
