Suggest Treatment For Diarrhea In A 2 Year Old Child

Question: Hi Doctor,
My 2 years 3 months old baby is suffering from stomach upset. I'm not sure, if it can be considered as loose motion/diarrhttp://www.healthcaremagic.com/web/images/ui/consultNew.pnghea. She was having light cough and cold since friday (5-Feb ), we have givin 3 dose of Asthiline Syrup and 1 dose of Sinarest AF along with dabur honitus. her cough and cold got cured by Sunday Eve. We have been giving her XXXXXXX warm water to drink.
But since saturday evening, she has been passing stool 4-5 times a day. Initially we thought, its may be due to cough/cold, but it seems my assumption was wrong. my daughter bowl movement is little slower compare to other babies. She passes stool once a day or once in two days normally. And now when baby is doing 4-5 times, its making me worried.
Last time she was treated for diarrhea in july-15 ( 8 months ago ). Later she suffered with severe constipation and rectum prolong happened 8-10 times in next 3 months. But she is ok for last 3-4 months and we are not giving any medicine to her for her constipation.

I have just told you history of my baby, which may be useful for you to suggest any medicine.
Current Situation:
i)She is passing 4-5 times stool, but 3-4 times will be very little( hardly a spoon quantity ).
ii) It is not watery but very very soft ( semi liquid mostly )
iii) We have started giving enterogermina tube since yesterday along with Z&D.
iv ) We are making her drink lots of water, giving her homemade soft food ( like kichidi/Idli/Daal/boiled rava ), coconut water and cow milk 1-2 times , NO Fruit juice ( she never likes it)
v) currently no cold, she coughs once or twice
vi) No Vomiting , She is passing urine regularly like earlier, but her stool color is little brownish ( like ragi powder color ), eventhough we stopped giving her ragi+dal powder milk ( First bite product).
vi) She is feeling hungry and will ask for water or bisicuits or some sweet food. We do not give chocolates but giving 1-2 biscuits a day.
vii) she seems to be crying holding stomach first 2-3 days but seems to be little Ok, after we started coconut water and given little boiled ajwain/Celery water on monday.

I am thinking to meet local doctor if I do not see any improvement or you suggest me to see doctor immediately. I am scared that she should not get into constipation problem again. My brother who is general physican, he had suggested me to start Nizonide-o ( Nitazoxanide + Ofloxacin ) but I have not started yet.
Please suggest on it. Let me know if I should give Colicaid or any medice for gas or pain on regular basis for next 2-3 days.

I have gone very carefully through the details provided. I understand your concerns but let me assure that unless the stool is blood stained, antibiotics (like Nizonide-O) are not required and the mainstay of management is adequate hydration. In other words, the loss of salt and water needs to be made up for and hence ORS (oral rehydration solution) needs to be given. However, I find that the problem could actually be a part of the problem of constipation which often alternates with each other. In such a case, proper fiber intake is very essential.

Please note that children of 1-3 years need 19 grams of fiber per day.

Enterogermina contains beneficial bacteria and so does yogurt or curd. So, you can give any of the two.

It is a good practice that you do not give chocolates. Continue giving biscuits, ajwain and celery water. Continue to give her ragi and dal. Give Colicaid only if there is unexplained crying or abdominal distension.

Monitor the urine output. If the frequency of passage of urine is normal, you need not get concerned and in such a case it would not be urgent to rush to the doctor.

Dear Dr. XXXXXXX

Thanks a lot for your words and time.
However I still have few queries and request you to help me, please.

1) Can Enterogermina cause constipation, I have started from yesterday and planning to give 1-2 more days. Please tell me, how long I should give once I started.

2) What is making us worried is that she is passing potty very little and it is not normal. Stool will be very soft ( semi liquid ) and color is definitely NOT Green or foul smell but color is not yellow. It is Ragi powder color. Is it somthing where we should get worried. However we have been giving her ragi everyday for last 3-4 months but stopped for last 2 days. Please tell, my wife is so much worried on it.

3) My baby seems to be prone to cold/cough and go to creche. So, I will be scared to give Curd/Coconut water/Banana ..etc . If she has light cough, can I give Dabar Hanitous Syrup or Asthiline's few dose. In General, if she gets cough, we give these medicine and she will be better. Can these medicine cause frequent motions now ?

4) We were told by few doctors that Cow Milk causes constipation, so we started giving Ragi+Dal milk. Does Cow milk really causes constipation, do we need to dilute it before giving to 2.3 years old baby. Unfortunately my baby doesn't like to have fruits much neither juice. So we will be worried, if we stop milk, it will be a concerns in her growth. However we are giving her Rice/Dal/Vegitables/Roti/Suji/Oats/Coconut ..etc so that she should consume sufficient fiber.

Humble request you to advice on above.

1. Enterogermina will not cause constipation. Give Enterogermina for three or four days.
2. You need not worry about the stool but the constipation needs to be taken care of and I have already mentioned what is to be done in that regard.
3. Giving curd, coconut water or banana will not make her catch cold. In fact, these would be beneficial when given in such a situation. Those medicines will not cause frequent motions.
4. Cow milk may cause constipation. More importantly, milk is just an accessory item provided that you provide the nutrients through other food items. Giving curd instead is much better. But it is not the presence of some item (like cow milk) that would give her constipation, it is the lack of adequate dietary fibers that will give her constipation.

Many thanks for your valuable advise.

She seems to be improving. But yesterday night she was restless and started crying loudly.
We were not able to guess reason but felt her stomach is tight. Then gave around 1 tea spoon Colicaid. Then she passed gas and she slept after 45 mins.

should you continue this for 1 or 2 days.

Could you please tell me , what should be correct dose for 2.3 yrs baby ( weight 14 KG ).

My wife suspect if she has worm in her stomach. I'm not sure but do not see any such symptoms as per my limited knowledge. She just ask for more sweet food, I meant she like sweet dishes more and bites her mother's breast sometimes very hard.

We have never given her any medicine for stomach worm yet, is it advisable to give at this age proactively . Do you advise her to see local doctor for it.

Her physical growth and weight seems to be very much normal to me.

We used to give her himalaya bonnisan earlier and later stopped it. Please advise, if we can start again or if you can advise any medicine, which will boost her immunity or help in digestive system.

It is good to know that her condition is improving. Colicaid Drops 1.5 ml should be right to be given to her. No problems that you have given her more earlier. Bonnisan can be given as an alternative to Colicaid.

Periodic deworming (once a year or once in a couple of years) is recommended in such children. Manifestations are not always present but craving for sugar does not mean that she has worms. Sugar does not give rise to worms. Putting contaminated items in the mouth does. I would advise refraining from giving sweets and junk food. If you want to deworm the child, you can give Syr. Bandy (Albendazole) 200mg or 5ml once and repeat the dose after a fortnight.

There are no medicines to boost immunity. Try giving her all sorts of fruits and vegetables, especially the colourful ones. They contain antioxidants, vitamins and minerals and are beneficial. They will help her build her immunity.
