Question: I have dysenteria with bloody diarrhoea which started 5 days ago with a high fever after a visit to Sudan. Now the fever is gone but I still have cramps and bloody diarrhoea. My doctor has taken a faeces sample to be sent for analysis. It will only be back in a week. In the meanwhile I am losing weight and feel weak. Do you agree it is better to wait and not take any antibiotic?

I appreciate your concern

Based on your description it appears to be a case of infectious diarrhea and its not appropriate to wait for antibiotics.

By one week time it would be too late to decide on the antibiotics. Stool routine exam including ova and parasite would require less time. Clinically if there is blood in stool, leucocytosis and associated fever with tenesmus then antibiotics should be offered. A good empiric option would be ciprofloxacin with metronidazole and diloxanide furoate. Fluid replacement should be done promptly to avoid dehydration.
