Question: I have a nephew and he is 38yrs old with bipolar disorder/depression what is a good medicine fore him? He is taking Risperdal 3mg BID, Gabapentin. My sister says he has anxiety attacks. My nephew has not talked all his life but able to say some words. He been having mood swings lately and lashing out hurting my sister. Can you pls advise.

I have gone through your query and understand your concerns. Risperdal and Gabapentin are good drugs and I assume that they have been considered suitable while they were prescribed. A number of factors need to be kept in simultaneous consideration while making such a choice. So, it would not be justifiable to make a choice sitting at a distance. Besides, it is very important to follow up the patient from time to time and to observe whether things are going as per plan and if dosage adjustments are required. For example, Risperdal is known to increase aggression and that explains his mood swings and rash behaviour with your sister. The solution would be in following up with his psychiatrist (who prescribed the medicines) so that he can modify the regimen suitably (after considering other factors as well).
