Suggest Treatment For Depression

Question: Dear doctor, my mind will be full of negative thoughts. I will always be worried that what others would be think if I do this /what others would feel if i do something etc. I want to have the confidence to talk freely without any shyness infront of people, to walk like a model with head straight while walking on street. I keep feeling that my dressing style is not good, walking style is not good, people would be noticing this, they would make fun etc. These days I am having some kind of depression (I will be happy & full of energy but suddenly I loose the energy level, I become moody. I feel as if I have worked a lot & feel tired!!! though I have just entered the office). I am having very low confidence level these days. Can u pls suggest some medicines to over come this issue (I have already contacted Psychiatrists for counselling but they have just guided me to be calm etc. This hasn't helped me in any ways because I will be calm but those mood swings spoil my day). Pls help. I know that u cannot prescribe online. Pls give the details of medicine which would help me with details. I will visit my family physician & show him your reply & request him to write those medicines.


I completed MBA & I am working in MNC. I am happily married (Love marriage). Have a lovely kid. I have bunch of good friends & I am happy socially, personally & professionally.

I have been facing this problem from past few months.

It started suddenly. I used to be a very energetic person with a lot of cheerfulness. I dont have any stress in office, neither in family or with friends. I eat good, I sleep properly.

Ya my friends & family members have noticed that I will be so talkitive & full of energy & suddenly I go down. They have asked like "What happened, u have become dull, look tired, Cheer up"

When I consulted psychiatrists, they told me to be calm, start doing yoga /meditation etc. I have been doing yoga & meditation from so many years. I always try to be calm but their suggestion didn't help me.

Well there were no situations like anxiety, stress, anger etc.. Mood swings all of a sudden. I go silent, loose energy & loose confidence all of a sudden. When I loose confidence, I wont be having confidence to walk with confidence (Straight walk), I wont be able to look into any one's eyes, I wont be able to talk with confidence etc.

Nothing like increased money spending like that & never thought about suicide. Never.

This depression, loss of confidence hasn't affected my personal, social & professional life much. I mean to say like during these mood swings, I would be a little slow in completing work in office, talking very less to colleagues. Thats all. It hasn't affected so seriously. In family, when mood swings, I keep quiet, though interesting topics /events are happening at home (While watching tv with family etc.) I just keep quiet, react only for name sake. etc.

My intention is to correct / treat this now itself before it goes worse or before it affects my social /professional /personal life.

I have gone through your question in detail and I can understand what you are going through.
From what you have stated it is quite clear that you are having a depressive episode. Depression can occur with regular stress and it can also occur biologically without any stress as well. You need proper treatment for the same. There are various effective therapies for depression like fluoxetine, venlafaxine, bupropion, mirtazapine or escitalopram and they are all very effective and mostly safe. You may also require some benzodiazepines like clonazepam or lorazepam for a short period like 2-3 weeks to reduce the anxiety and help with your sleep.
If you are averse to the drug therapies then there is also an option of cognitive behavioural therapy. This therapy is taken by either a psychologist or a psychiatrist in which the therapist identifies impaired cognitions (Thoughts) and try to correct them. They are 15-20 min weekly session and there should be around 8-10 sessions to help you with your illness.

Thank you. before going for behavioral therapy, I would like to go with the medications that u hv suggested.. can u pls give the complete details like - name if the tablet, what mg., when to take & for how many days.. what would improve by taking this tablet. Thank u. once u help me with this, I can go to my family physician (He is a general practitioner) & show ur prescription & I Wil request him to write those medicines.

Sorry for the delay as there were some technical problems.
You may consider starting with Fluoxetine 20mg once a day in morning after breakfast along with clonazepam 0.5 mg at night after dinner. These are prescription drugs only and you should obtain a prescription from your local doctor.
You may get them prescribed by your doctor.

Doctor, thanks for replying. U didn't reply to my question that how long should I take the medicines, what these tablets will do or help me in. By knowing this, I can evaluate whether any changes are happening during the course or not. Also, I am allergic to SULFA.

Hello
The duration of treatment depends on when you get 100% improvement in your symptoms. Ones you get that, the medicines need to be continued for 6-10 months there after.
The first change that you would notice is reduction in anxiety and the last that would go of is amotivation.
Its not a SULFA drug.
