Suggest Treatment For Depression

Question: Hi,

I am 29, female.

I am not sure if I can call my condition as depression. I went through love failure and he is haunting. I am fine and happy when I dont remember him. But when I remember him, it is giving me a lot of pain and taking a lot of time to shift my focus onto something else. I tried a lot not to think of him and remember him. But everything lasted only for a short time and again he comes back into my mind. I finally concluded that it is out of my control to keep his thoughts away. He is sitting on my head constantly. Every time I think of him, I am crying. I dont want to spend my life like this. I am unable to focus on my work. I want to come out of it completely. I want to focus on my career.

I tried Nexito 10mg (not sure if my condition is depression) previously but I noticed side effects of it (very delayed orgasms) I stopped it and I read that almost every anti-depression tablet have this or some other side effect. Any tablet without these side effects?

Please suggest me a way to come out of it totally and forever.

I have thyroid and PCOD.

Brief Answer:
Yes Bupropion is there and counselling can help

Detailed Answer:
Hi XXXXXXX

Thanks for using healthcaremagic.

I am sorry for your break up.After break up normally person feels this type of problems. However duration of such thoughts is 1-2 months and gradually they go away. When these thoughts persist for long or even during this 1 month they are so sever to disturb your routine life, treatment is needed.

Our mind has no capacity to forget, but yes we can accept our bad incidents in life, digest it and can put deeply inside subconscious mind.(Example:In computer if we use shift+delete keys, file deleted forever we can not recover it , our brain do not have this facility, but yes of course brain can delete it and put in to subconscious mind like recyclin bin in computer from where we can restore.)

These type of haunting thoughts can be reduced with medicines like SSRI(Nexito). But you are having side effects with that, so I can suggest Bupropion generally in 150 mg to start with. It has no sexual side effects and work well in young patients. You can discuss with your psychiatrist for that to start.
Your condition may be depression if you have sad mood most of the day, lack of interest in pleasurable activities, sleep disturbance, decrease appetite, hopelessness, worthlessness and so on for more than 2 weeks.
Diagnosis can be confirmed by psychiatrist.

Another option which can be applied in alone or simultaneously with medicine is psychotherapy.
One technique called Cognitive Behaviour Therapy(CBT) is quite helpful to change your though pattern and decrease automatic thoughts.
Another is short term Psychodynamic Psychotherapy.
If you are good motivated, psychotherapy can work excellently in this type of problems.

To help you further, I just want to know, " Duration of your break up and your thought frequency has decreased, increased or remain same since beak up? and some detail of break up if feasible "

You can continue your thyroid and PCOD treatment if any going on. If you are stable in these two problems of thyroid and PCOD, there is not much relation to your psychological problem as your thyroid report is normal.

Thank you very much for your advice.

Break up happened more than 8 months ago. Thoughts were intense for more than 3 months. It is only recently that I am doing fine. Even now when I think of him, I cry and remain dull for half a day and I lost that fire in me and not being very active.

Yes, I have sad mood most of the day, lack of interest in pleasurable activities.

sleep disturbance, decrease appetite, hopelessness, worthlessness are not there. I used to be very ambitious. Even though I dont feel hopelessness, I lost that fire in me.

I didnt consult any psychiatrist as such. I asked a doctor in a website and she suggested Nexito. But once I noticed the side effect, I stopped it thinking I will handle it myself. But I realized I cant. I would like to continue the treatment with you.

I see side effects for every anti depression tablets. I found the below for Bupropion. I am ok to take them if they persist for few days and will not have long term effects. Kindly suggest your opinion on this. and how do I start the treatment.

I have generic questions about such tablets.

1. I am fine when I dont think of him. Problem comes when I think of him. So ideally what I should do is not think of him. How can anti depression tablets solve this? They are basically to elevate mood. So even though I take them, he is still there in my mind and I can still think of him.

2. How many days should I take the tablet and will everything be ok after I stop. Or is it like once I stop, I get back my problems again.

I take vitamin supplements of A, C, D, E, b-complex, calcium daily. And metformin 500 mg daily twice.

Thank you.

Brief Answer:
You need counselling

Detailed Answer:
Hi XXXXXXX XXXXXXX
Thanks for follow up question.

Practically and frankly speaking self medication without consulting doctor in personal for such complaint is not proper form medical point of view as well as legally.

However Nexito(Escitalopram) is one of the good drug without significant side effects except sexual side effects and used mostly in young population who do not have concern for sexual side effects. So it is ok that you have taken on your own.

Bupropion is also good medicine , specifically Bupron XL 150 for depressive symptoms and very well tolerated.
Any medicine allopathy have mentioned side effects however it is not so that every one suffers from that. It depends on individual physical and genetic make up.
Anti anxiety medicines like etizolam or clonazepam can be given after detail evaluation for short duration of 15-20 days for immediate relief from your thoughts.

I your case after 3 months you are somewhat better so you are not in sever problem.
Medicine will not work to forget him, but medicine will help you to be more active and able to concentrate in your work by reducing sadness severity.
As time will go by nature you will accept the fact that your relation had ended up and nothing to do with him now and you will not feel sadness or cry even if you remember him. After that you can stop medicines. So medicines are there to prevent worsening or can say to prevent XXXXXXX episode of depression.

If I were your treating psychiatrist, I will go for psychotherapy only as I mentioned in previous answer because already 3 months had passed and your routine life is maintained though some depressive features are there.

So please consult without any hesitation some good psychiatrist or clinical psychologist who can spare time for psychotherapy and if you wish you can be helped with medicines also as I mentioned after consulting psychiatrist.
I am sorry that detail counseling not feasible on this forum.

All the best and wishing you early stability,

Regards,
Dr. Chintan Solanki.
