Brief Answer:
consult psychiatrist.

Detailed Answer:
Hi dear,

First of all your sister needs help of psychiatrist.

Once she is out of depression and able to understand her condition she will take care of her medical illnesses other wise she has no insight of her medical and other illnesses.

Also you told that her IQ is low, but how you comes to know?
If you think that consult a psychologist for detail IQ assessment.

Diabetes is medical illness which has many life threatening complication and for which she must take treatment either by forcefully or in food.

For assisted place you first make good treatment plan than with help of doctor you can put her in such place.
If she is not ready than take her to psychiatrist forcefully.
Awaiting your response.
