Brief Answer:
get a serum beta hcg test done

Detailed Answer:
Hello,
Thanks for the query to HCM,
If you have been sexually active and have been active during your pvulation period , then it would be wise to get a serum beta hcg test done as on date to rule out pregnancy first. However if negative then the symptoms can be attributed to PMS symptoms.
The discharge from breast nipples indicate high prolactin levels due to possible hyperprolactinemia which can invariably cause irregularity in menses, so a serum prolactin level investigation is mandatory. This can be further controlled/ treated with a course of dopamine antagonists. Please follow up with serum beta hcg reports.
Regards
