Question: Hi Doctor, I am asking this question for one my friends daughter, she born on 17th March 2015. She had some breathing problems today and they took the baby for checkup. Doctors advised for some x-ray and 2d Eco and came up with a result small os-asd ( 3-4 mm) LT -> RT shunt. What is the best possible action to take, They are in a District Head quarter with reasonable access to Medical facilities.

I appreciate your assistance in providing advise.

Brief Answer:
I won't be overly worried for 4 mm of ASD

Detailed Answer:
Hello

Thanks for the query

I would like to give you some idea about the congenital heart defects so that you can understand the condition well. In congenital heart defect there can be left to right shunt where there is no mixing of deoxygeneted(impure) and oxygenated(pure) blood while in right to left shunt there is mixing of deoxygenated and oxygenated blood and thus causing bluish discoloration of body. This is the reason L-R shunts are considered to be less severe than R-L shunts.
Out of all the left to right shunts the ASD(atrial septal defect) is the most benign one.Even a normal person can have 3-4 mm of ASD as very rarely this does produces any symptom. The main aim of management of this size ASD is follow up with echocardiography.
It has been seen that OS ASD of size 3 mm closes in 100 % of cases after 1 year without any treatment and does not produces any symptom. The closure rate of 4-8 mm ASD is 80%.
After knowing all these facts I won't be overly worried for OS ASD and would like to find out other causes of respiratory distress like viral or bacterial infection if any.
I hope this answers your query well.
If you have any more doubts I would be glad to solve it out.
If you don't have any query then please close the discussion and rate it.
