Question: 53 yr old women running 103.5 fever taking motrin fever dropping then back again 103.5 went to e r room took urine signs of uti infection but on cipro sent home .now after three days shaking ,freezing cold with a normal temp no fever . went back to er room took blood took X-rays. now they put her cephalexin 500mg 3x day also asked if there was anything going on with kidneys they said something wasn't perfect but didn't say anymore so they discharged now 2 days later freezing cold no temp. sweats, confusion,hot, cold .do know what to do

It looks like you are suffering from urinary tract infection which has affected the kidneys as well, it is termed as pyelonephritis. Here is what you should do

1. Repeat urine routine and urine culture. Urine culture will tell us which bacteria is growing and which antibiotic it is sensitive to
2. Get an ultrasound of the kidneys done to see for abnormality like stones, obstruction ,etc
3. tab acetaminophen will help the cold and shivering
4. in urinary tract infection it is important to get a renal function test done to know if the kidneys are affected
5. keep a watch on the amount of urine you are passing per day and watch out for swelling of the feet. Passing less than 750 ml of urine and swelling of feet calls for admission and treatment with injectable antibiotics

thanks doctor very helpful . just a question. when you have a uti or kidney systems sweats,fever,chills, confusion are common symptoms

You have asked a rather difficult question. It is true that UTI and stones can cause the symptoms you have enlisted however these patients will have a predominant symtom of lumbar pain or pain abdomen. Presence of fever, chills and confusion will be toxic requiring hospital care as an in patient

i think the issue can do something with kidney .what would a ketone strip show anything helpful?

dr.we have chill every 4hrs for 5mins then goes away comes back every 4hrs taking 500 mg 3x day cephalexin plus tylenol- motrin no symptoms of lumbar or abdom pain what do you think doc?

Chills is a sign of systemic response to an infection. It is normal to have chills when one has an infection, it is our body immune system fighting the pathogen
