Question: I have a constant chronic headache for many, many months now. It feels like someone is kicking my forehead with a cement boot.

- did you experienced headaches during sleep?

- any other drugs used?

- where exactly is headache located?

- any other symptoms besides headache such, nausea, vomiting etc.?

- problems with your menstruation, or are you in menopause?

My reasoning for using anti-migraine drug is that I figured it is the highest level of medication that I could get for headaches over the counter.

My blood pressure runs rather low: 1-week average = AM (before meds) 95/57

Osteoarthritis of the lumbar region, fibromyalgia, Major Depressive Disorder, also experiencing tingling in both hands (just hands, does not go up wrists or arms)

There are no factors that ease the headache, they are chronic at all times. I have tried ice on the forehead, ice on the back of my neck, heat on the back of my neck, quiet, darkness; nothing changes the intensitiy of my headache.

The headache is constant. I have my headache until I finally fall asleep, which usually takes a couple hours upon laying down in bed. I wake up, everyday, with my headache, it is at chronic intensity upon rising.

OTHER MEDS:
Effexor XR (150 mg)
Wellbutrin (150 mg)
Klonopin (1 mg)
Omeprazole (40 mg)
Topamax (50 mg)
Etodolac (500 mg)
“AS NEEDED” MEDS:
Tramadol HCI (50 mg) – limit 6 times a day

My headache is located exactly in the middle of my forehead. It feels like someone is constantly kicking me there with a cement boot.

Other symptoms besides my headache would be some sore muscles in my shoulders, but no nausea or vomiting, etc.

I am in menopause.

Sinusitis should be evaluated.

Detailed Answer:
Hello again and thanks for the clarifications.

Since the location of the headache is middle of the forehead, most probably it is caused by sinusitis, or infection and blocking of paranasal sinuses.

Initially, x-rays of the sinuses is needed to evaluate correctly this possibility.

If doubts on x-rays, CT scan of the head and sinuses gives a lot more information about this condition.

If sinusitis is excluded as diagnosis, the second possibility that may cause your symptoms is changes of neurotransmitters levels due to use of Effexor and Klonopin.

In my opinion, you should discus with your Doctor about assessing sinusitis by examination and tests first, if sinusitis excluded, changes in doses or drugs you are taking are needed.
