Question: Hi doctors,
Still pain in my chest, i will say lol , because i always ask about the same thing, but my chest is painful...in the morning until i not have smoked my first cigarette i feel like pressure and pain in my lungs...then most of my pain in chest is from left side pectoral to armpit and behind my left shoulder blades...i have done cardiac test without problem, lung test without big problem small atelectasis, full echo of abdomen nothing except some fat nodes, and im on treatment for gastric reflux but still not improvement for the pain. What else can cause it??i have more pain during exercice and stress , can it be muscular or come from bones as well?
is it normal for a men to feel a kind of mass under my left nipple, i have checked on the right one and dont have it...i will say its 3 on 3 centimers and not painful...what it can be?
I still have pain in my chest mostly left side and in my left arm, i still consider than it can from anxiety or muscle but how i can fix it???

Hi.

You mention that you have got chest pain on the left pectoral and in the arm pit that is increased by exercise and that you smoke. You say that this pain is lingering. However, you have taken all the cardiac and the lung function tests and have come clean of any disease there.

It has been found out that you have reflux. In clinical medicine, the most common cause of the sort of problem that indeed you do describe is reflux of acid secretions. Now, do note this, I would ask you to change your medications as regards the reflux problem. Maybe you can try esmoperazole and it's combinations. This works for persons who do not respond as well to gaviscon and omeprazole.

Yes. The nipple that is enlarged on the left side is due to prolactin level increase. This is an unwanted side effect of medication. It can also be physiological gynecomastia. It is certainly not male breast cancer, else you would have been facing enlarged lymph nodes, weight loss and other issues by now, and since the doctors have examined you there is nothing to worry.

If it really bothers your masculinity, then you need to go to a doctor and get a prescription for a drug called as bromocryptine. This shuts down excess prolactin and causes the nipple to return to normal. A short course will suffice although not neccessary. Surgical correction is not warranted here.

I'll give you some quick relief from this pain without painkillers. Note that you smoke and probably have chronic bronchitis. Smokers get irritation of the pleural sac of the lung that hurts in exactly the areas that you describe. When you mention that your ribs hurt too, these are called as accessory muscles of respiration. Note this.

You need to see a doctor for a low dose of a combination of chlordiazepoxide and nortryptiline. This combination relaxes the muscles of the chest and dialates the bronchial musculature relieving the pain. Nortryptiline has the added effect of relieving the low mood associated with chronic pain conditions too. These two drugs have an additive effect and are commonly prescribed together.

Doctor, for you if have i done all cardiac examinations like EKG, EKG Stress test ,echo i can really exclude all cardiac disorder? for lung i have maked only X ray and for abdomen Echographia...
ITs mostly on my left part of chest i have pain between the chest and arm pits , its like muscular pain , but can it be related to my heart?

The only way to decide whether you have a heart problem is by all the investigations that you have done that prove thoroughly that you do not have a heart disease in an active form.

The commonest reason for such pleuritic pains and not cardiac pains is irritation either of the lungs or of the esophagus and stomach.

I am certain, after carefully examining your history, that you do not have heart disease. It is for sure.

I had requested you to stop smoking. Please try to stop smoking for a week and you'll find a difference.

Hi Doctor,

I have done PFT like u did ask me, and everything was nearly normal i was borderline he told me but im sure i could have spit more strong in the machine, the tube was so big and the nurse dont give me right instructions, its happen same mistake last year, doctor believed i was doing asthma but i do an other test PEAKFLOW with other doctors and he told me everything was normal... i still have pain on left side of my sternum, doctor also think about Costochondritis, i think its maybe what i have but he gives me Arcoxia....Being anxious about medicine, i have maked some researched on that medicine and apparently its more bad than good, i mean give you more bad effects than good results...In France they are talking about delete than medicine of market because too many complains and apparently its not good for the heart ( im already paranoiac about my heart ) so there is a healthy or way to treat costocondritis without use heavy medications??

Hi!

The drug of choice in your case, according to me, is Propranolol. You need a low dose of it (10 mg) regularly(you'll be needing a prescription from your local physician to get it). This medication will calm your heart and increase your concentration. You'll feel relaxed, fresh.

Yes, I sympathize with the fact that you had a rough time with the PFT's. The RN should have been more humane. These devices are unfamiliar to people and the person taking the test needs to be counselled well.

Your breathing rate will be lowered after taking propranolol and this will lessen the pains of pleurisy. Or perhaps pleuritis.

So, try this medication. Do not depend on internet reviews written by persons with probably vested interests. And as for France, they have a different culture and their approach to illnesses differs from what you are probably used to. I would not take internet reports seriously enough unless I was speaking or writing to a doctor, even if it were online, just as you and I are communicating now.

Get better and be happy. You can indeed quit nicotine. I am sure of this, why don't you try nicotine chewing gums or lozenges? It'll certainly work.

This is because the day you stop smoking, is the day you'll be free of these lung pains, regardless of the diagnosis.

Thank you Doctor,

I will follow your advices at least really reduce my cigarettes and switch to Propanolol i will keep you updated...for you its not possible it can be a cardiac pain after all the exams i have done even the pain is stronger when i do exercice and after meal?the worst is on evening i dont know why and i would like to know more about it...i feel like a cramp and burning under my sternum and really stressful feeling...i will continue exercice also by the way and i have planned to visit a chiropractor next week in order to put my back properly , one of my doctor thinks its costochondritis or maybe a coronary disease but he says me it will be really surprising after all the test i have done but it can happen and the only way to see that will be to do coronography which i dont want to do for now because its invasive procedure...Thanks

If you experience pain in the chest on exertion and it periodically visits you, this means that given the facts that all the cardiac and pulmonary tests being negative, you have an inflammation of the lungs. If a smoker develops these exact issues, the choice would be to live with the pain, or to endure the process of quitting smoking.

I would kindly ask you to stop cold turkey with smoking and wait and watch. More so, than medications, you might find that your pains diminish altogether.

Remember, "Never take another puff just for today".
