Brief Answer:
This is mostly cervical spondylitis

Detailed Answer:
Hello,
I have studied your case . it is mostyl cervical spondylitis. In this condition there is disc herniation in the neck which compresses nerve and causes radiating pain in shoulder , elbow and hand. I would suggest you to get MRI of the cervical spine to confirm the diagnosis .MRI will also help us to decide further management .
in my patients with such problems I have following suggestions.
1. As you are telling me that you are taking physiotherapy. I would suggest you to take intermittent traction to neck. This will distract neck bones and lead to decrease in pain.
2. Avoid using very high pillow, working long time on computer, very heavy workout or watching TV for long hours.
3. I suggest tablet Pregabalin and Methylcobalamine . Both are good in nerve related pain.Discuss about this your doctor.
I hope these measures will help you in your pain. Let me know if there is any other query.
Thanks.
