Suggest Treatment For Candidal Infection

Question: Had sex with my girlfriend while she was undergoing periods, after a few weeks she complained of irritation and discharge from her vagina and i also developed a couple of tiny blisters on my penile head and burning sensation during urination. Her gyneac diagnosed her with viginitis and also suggested me to take the medication called FAS 3 kit. Please help.

I can understand your concern for irritation and discharge in your girl friend and also tiny blisters on your penile head with burning sensation during urination.

After analyzing your query it appears that both of you have developed Candidal infection. It is quite common in both male and females to get itching and burning sensation due to Candidal infection.

It is due to the fact that you did sexual intercourse during menses which can expose the partners to Candidal infection. It is basically first manifested as mild to moderate burning along with itching and later on is accompanied with mild curd like discharge in females.

In males there is no discharge but there can be irritation and burning along with very small tiny blister on penile head.

In all your kind of cases I suggest to interrupt sexual intercourse for 1 week and observe if any discharge is present. In case if there is no discharge then simple maintenance of hygiene along with sexual abstinence helps in early resolution of the problem. But in case if there is increase in symptoms along with discharge then both partners should get treated quickly.

Both the partners I suggest following treatment in this situation.
For females: I suggest vaginal pessary or cream with clotrimazole or sertaconazole and ornidazole to be applied or inserted in vagina for 5 consecutive days along with fluconazole tablet in the dose of 150 mg on alternate day basis for 2 weeks.

In males: I suggest plane sertaconazole or clotrimazole cream to be applied at night time in very thin layer so that it should not be visible to the naked eye. I also give fluconazole tablet in the dose of 150 mg on alternate basis for 2 weeks.

For itching, I prefer to give Des-Loratadine for two times if itching is high and for one time when it is reduced for 7 to 14 days.

The treatment prescribed by the gynaecologist is quite fine and both you and your partner should take the FAS 3 kit. Additionally I suggest you to take treatment as suggested above to take care of infection in yourself.

Drink plenty of liquids to flush your urinary system and to reduce the burning sensation during urination.

Do not worry both of you will be fine soon.

The blisters are gone but the penis is still quite red and tender to touch. Burning is also quite severe. My fasting blood sugar is normally around 100-102, any connection with the infection?
Also just wanted to clarify the dosage. do you mean 1 tablet a day, alternately for 2 weeks?

Regarding your further queries:

The penile skin is in recovery phase and therefore the blister have gone by now. Had the blisters remained or if you had found more of the discharge then we would have considered an increase in infection..which is actually not. Presently there is only tenderness which is quite normal in healing phase.

More specifically this is related to the hygiene especially in uncircumcised males. It is more common to find moist area at glans penis and frenulum (skin covering the glans penis) area which favours the growth of fungus.

The fungus can be grown directly in an individual or can be more commonly because of infection from the partner. So in case of unprotected sexual intercourse if the area is not washed properly or later on if the hygiene is not maintained properly anybody can get the infection again. So the same thing is in your case too.

Three are further of the suggestions below: -

-     Wash your penile area for two times in a day. You can use a mild soap to wash the area. Wash it very gently but properly. If you are uncircumcised then it is quite important to retract your skin fully backwards and then wash the area.

-      After every sexual intercourse too, wash the area properly. I personally suggest you to use protection in form of condoms during every intercourse. Though you have to abstain from intercourse for dew days and be very gentle in next few instances.

-     Wear cotton undergarments. Change these two times a day. Do not wear any synthetic or tight garments.

Your fungal infection is not related to blood sugar as your status is quite healthy in respect to the same. If the blood sugar fasting is above 120 then we consider it as important cause and control of sugar becomes mandatory in these cases. In your case you need to do any active intervention for control of sugar.

Do not try to touch the genital area repeatedly. I quite often see my patients who touches and repeatedly examines the penile area out of anxiety and this causes more redness with further aggravation. So kindly do not try to touch or see the area repeatedly.

The dose of fluconazole is 1 tablet of 150 mg on alternate days for 14 days. It means you have to take 7 tablets in 14 days on alternate day basis.
Do maintain a good hygiene and be vigilant next time to avoid such kind of episodes. You wile b total y alright soon. Drink plenty of liquids and flush your urinary system to avoid any urinary infection too!!!

A very good morning to you!
This would be my last follow up question, and sincere thanks again for your detailed explanation of my condition. after this i shall close the conversation and rate it! (Which would be definitely highly satisfying)
Getting back to the subject,
Just for your information after the medication (FAS 3 and flucanazole) i have developed a whitish tongue and those dots around my lip (maybe they were there before i might have just noticed) , pics attached for your reference. is this oral thrush???? Honesty i am scared now. really scared. do i need to get tested for something bad? From all the things i read on the net! Also i developed a new red dot on my glans penis. I am following your recommended medication, penile burning has reduced but still there, especially after passing urine. I am also applying clotrimazole ointment before bedtime.

I have gone through both the pictures and after analyzing these with best diligence let me reassure you first that that this is neither related to oral thrush and nor to any type of sexually transmitted infection of any sort.

These simple glands in the oral mucosa which are more pronounced and are present since long. It is the only part that you noticed these in day or two because of anxiety and therefore perceived these for first time.

I fully reassure you that these are very simple prominent glands which all of us have. SO rest assured about the same.

For the part of penile issue: the things will be aright soon and as you can notice that it is getting better day by day. You may have one or the other eruption which are simple proliferation of the glands after the infection. So just maintain a good hygiene and do not attempt any intercourse or masturbation for few days.

I additionally suggest you to not read anything related to your condition without an expert suggestion. Internet is full of knowledge but you don't know which thing is applicable to you. So it becomes a big stress when we find something unusual which might on might not be related to your issue.
