Suggest Treatment For Burning Micturition While Having Diabetes

Question: Iam also newly attacted diabite. Iam using glyciphage 500 from 2 months after breakfast and after dinner. There is control in morning and night but after noon i got above 230 after lunch. Iam also have thyroid tsh levels 13 and normal t3andt4. Iam suffer urinary burning from 2 months.so many anti biatics are used for urinary problem will you please suggest me.

Dear Sir

I have gone through your question. Here are my suggestions:

1. Instead of taking Glyciphage (plain) twice a day, I would suggest you to take Glyciphage SR (sustained release) in dose of 1000 mg at bedtime.
2. Since your postprandial sugar is high , I would advice you to take T. Voglibose 0.3 mcg twice a day immedatley before lunch and dinner.
3. Your TSH is high and your T4 is normal which means you have subclinical hypothyroidism (SCH). For SCH with TSH more than 10 mIU/ml we generally suggest treatment. I would advise you to take T. Eltroxin 50 mcg once a day in the morning 45 min before breakfast. Please repeat Free T4 and TSH 6 weeks after starting medication
4. For your burning micturition, I would suggest you to get done urine culture and sensitivity testing. The antibiotics can be changed/started after the report.
5. I would suggest you to exercise 30 min a day
6. I would suggest you to take 1800 Kcal/day diabetic diet. You can consult a good dietician to make you a diet chart for diabetes.

7. Please get done the following investigations and you can revert back to me after the reports:

a) Lipid profile
b) HbA1c
c) Urine routine and micro
d) Urine C/S
e) Free T4 and TSH (After 6 weeks)
