Question: My 16 yr old son had visible blood in his urine on 7/14 during football camp--was not hit; urinalysis performed Monday with results of: RBC,urine 418 (range 0-5); slightly elevated CPK. Urinalysis results today are Hemoglobin Small (1+), Bacterial Small (1+), and RBC/HPF 6-10 (range of 0-2). Have an ultrasound in the morning. Does this appear to be something serious? He was on the medicine Dapsone from Jan-Jun 2016 for acne, but came off 2 weeks ago, because it was causing anemia.

Brief Answer:
Hematuria.

Detailed Answer:
After an unusual exertion, its known to cause hematuria ( blood in urine) in some people. They have the tendency for such things. As your son had a visible blood while on play, its possible. Its also possible that the reason might be simple.
As the ultrasound report is pending , you will get a confirmatory report after that. It will show the presence of a stone or a rare cause blood due to any " lesion ( abnormality) in the urinary tract. Also, the presence of bacteria suggest that there is a possibility of urinary tract infection (UTI) which is the most common cause of blood in urine.
As such, boys are not supposed to have a UTI unless there is some abnormality in tract for example Presence of a small stone( which can get passed of unknownly).
You also can get urine culture done once as the bacteria are seen in routine urine tests.
