Suggest Treatment For Bloating, Dyspepsia And Nausea

Question: Hello. I had my gallbladder removed a number of years ago. No stones were present. Ejection fractions were 9% It was removed after 5 years of symptoms including extreme bloating, dyspepsia and nausea brought on by the pressure following eating. Within a year after the surgery, I landed in the hospital for several days with what was finally determined to be a stone in the common bile duct. Horrific experience, they believe it was passed before the ambulance arrived. I then had several years of fairly good results. For the last number of years, however, i have had return of many of the same symptoms. It continues to be intermittent. About the time I decide to see a Dr, it improves. I have had 2 biliary type attacks since the hospitalization, one of which was extremely severe. Otherwise it is constant bloating, diarrhea and gas in my stomach following eating. The only other thing of mention, would be an overnight stay at thre hospital for a lacerated pancreas from trauma. I was released clear. All blood work has been normal, including several months ago. This has been an issue for years at this point. On again, off again. I am tired of feeling badly. Now I am dashing to the bathroom after eating anything this last week. It has been much more troublesome as of late.

Brief Answer:
Possible Pancreatic diarrhoea

Detailed Answer:
Hello, Madam.

I can certainly understand your concerns.

i have worked through your query in detail.

You possibly seem to have pancreatic diarrhoea.

This requires further testing like ERCP and Stool fat studies.

ERCP require an experienced Gastroenterologist to preform the procedure.

Pancreatic Enzyme supplements may help to reduce the frequency and nature of the stool

Further specific management is largely based on above test results.

Post your further queries if any.
Thank you.

The diarrhea is not the most consistent symptom, though it has been more of a concern off and on the last year or so. Comes in cycles. Tehran most consistent thing is everything I eat seems to immediately turn to bloat in my stomach. Pressure, discomfort often times the point of nausea secondary to the pressure. After 20 minutes or so, I will start to belch multiple times and it will begin to subside. Don't get me wrong, the diarrhea is an issue too, just seems to come in cycles. Don't always have it even when I have the gas pressure in my stomach.
Is it possible that I am continuing to develop stones or sludge? I am almost certain that was the case with the severe biliary attack 2 years ago. Almost identical to the one post open requiring the 3 day stay. Would the pancreatic issues be secondary to some sort of obstruction? IE stone etc?
Thank you

Brief Answer:
Obstructions are more likely

Detailed Answer:
Hello, Madam.

Yes, indeed the pancreatic issues could directly relate to the obstruction of the pancreatic ducts secondary to Biliary sludge.

The nature of symptoms in your case are directly pointing to disease of biliopancreatic region.

Hence its advisable to get the tests I recommended in my earlier query.

Once the test results are out , further management can be planned.

Post your further queries if any.
Thank you.
