Question: Hi Doctor,

2days back (21st june) I had sex in macau @11 pm and I used durex latex condom. I had sex 3times in 3 hours. On 22nd june@7pm I realised some blisters (phunsi) on my left and right thighs.left had around 12 and 2-3 on right thighs. I am very depressed that I might had some std. Can you please let me if am having std or not. Pfa the image of phunsi. also I am having slight headache now.

I have gone through your query and i have also reviewed the image.

I can see that you have multiple red, raised bumps some of which are pus filled.
This seems like a simple bacterial folliculitis rather than an STD.

If I was the treating doctor I would have suggested an oral antibiotic for a week e.g either Amoxycillin or Cephalexin.
Oral antibiotics are prescription drugs and should always be taken under supervision of a doctor.
I suggest that you talk to your doctor for the needful.
Topically you may use an OTC topical antibacterial e.g 2% mupirocin ointment twice daily.

I had read lot of things over Internet which includes herpes and hiv. Do u think it can be herpes. Also what if I left these rashes untreated because gradually these are bursting and I am not having any pain or itching. It just when I press these muld pain arise. Do I consult my local doctor or let it remain untreated. Can it be recover on their own. Should I go for std tests. Please suggest.

Ideally you must use a topical antibacterial cream, if not oral antibiotic.
Even though your sexual contacts were protected but as a protocol you make undergo a screening tests for all STDs at 4 weeks.

Hi Doctor,

What test I need to undergo. These bumps are not painful and not itchable. Also some of them are getting bursted on their own, do u still want to consult dermatologist. As per ur advice I am taking amoxyllin amd ointment. do u still want me to consult dermatologist. Can we relate this skin problem to any of the std?

Hi.

A full STD screen at 4 weeks...
-Screening test for Hiv antibody along with P 24 antigen detection.
-A VDRL test for syphilis at 4 weeks.
-ELISA for Hsv type 1 & type 2, IgG and IgM antibody.

Amoxycillin should be fine and I suggest you to continue with it along with topical antibiotic cream.

The bumps in this Image don't look like an STD.
They seem more like a simple bacterial folliculitis and should be fine with the Oral antibiotic that you are taking.

Even though the bumps in your case don't look like those of genital herpes but a point to note is that, a condom does not guarantee 100% protection against Herpes Virus!!

Hi Doctor,

yesterday I fixed appointment with the doctor and he said its caseof superficial follicitus and had recommended minoz 100 for 20 days along with 2 ointments. I had taken vaigra 50mg without any recommendations on 21st. Now I am having severe headache. What should be done in this case. From 2 days I am taking saridon what should I do further To avoid these headaches.

Headache could be minocycline induced Or it could be tension headache.

Hi.

I don't think that headache is a side effect of Viagra (Sildenafil). However, Minocycline (Minoz) has headache as a well known side effect (Pseudotumor cerebri).
You should immediately discontinue minocycline. I don't even feel the need to start Minocycline.
I think your folliculitis should resolve on Oral Amoxycillin and topicl antibacterial which you were taking earlier.
You can take Paracetamol(Saridon), twice or thrice daily for symptomatic relief from headache.
Your headache could also be due to stress/ anxiety associated with the fear of STD's i.e tension headache.

Hi sir,

Thanks for your prompt response. I had checked my bp which was 100/150 and pulse rate was 138. I never had so huge pulse rate and bp. I checked my bp twice a week and always it comes around 72/110 . I don't know whats causing my bp to rise. from last few days I am feeling restlessness and anxiety towards std. please advice what should be done further.

Your BP values seem unrealistic to me. Value of 100/150 mm of Hg would be devastating for your heart!!. It is too high diastolic reading!!
Even 72/ 110 mm of Hg is unrealistic!! Too low systolic reading and too high diastolic reading!

Headache can happen due to high BP. Initially, I didn't consider high BP as a cause for your headache, since high BP is quite rare at your age of 25.

Anyways I suggest that you get get your BP properly measured by a nurse or practitioner and if comes out high, take appropriate treatment for it.

Hi sir,

Sorry for argument but I read somewhere that ideal bp is 80/120. So why 70/110 bp is abnormal. Please enlighten with your supreme knowledge. Amd I check it twice and it came out to be 100/150. I had also perform ecg 2 hours ago . Pfa the image for that. Please let me know what could be done in this case. Can this is being abnormal because of my streas and anxiety.

Clarification regarding BP readings

Hi.

Normal human BP is 120/80 mm of Hg. The upper reading is systolic and the lower reading is diastolic. It is not 80/120 mm of Hg.

Anyways, the confusion here is probably because you flipped all your BP readings vertically!

So actually your BP readings were 150/100 mm of Hg and 110/72 mm of Hg which are understandable rather than 100/150 and 72/110.

High pulse rate is probably due to anxiety.

I guess your headache too is most likely be due to stress if not due to minocycline. Anyways, I am not in favor of Minocycline.
Amoxicillin and topical antibiotics would be good enough for your superficial folliculitis and it should be fine within a week

I got my bp checked. It came 140/90, 101/145 and pulse rate was 120 both times. Can std Increase pulse rate. I am terribly frightened so please let me know what should be done in this case.

You need to relax. You don't have an STD as of now. You just have a superficial folliculitis.

Your high pulse rate is most likely due to anxiety. Your BP on two occasions i.e 140/90 and 145/101 respectively, is slightly higher and that is most likely also due to anxiety.

You just need to relax. There is nothing to worry.

Hi sir,

I am having tension that I am having hiv. I am having a slight red rash on my chest. Also am having heaviness in brain. Is there a test which can detect immediate hiv rather than 4 weeks.

These two tests are the earliest to become positive in an infected person. However these can be taken earliest at 2 Weeks.
