Question: 22nd may 2016 i got back pain and fever 100.4 F,,,,after 6 days 28 th may i got fever again 100.3 F..i consulted doctor,,he gave me tests for typhoid and jaundice,,,my widal test was +ve 1:160 and total bilirubin of 1.24,,again after 6-7 days i got fever of avg 100F on 4th and 5th XXXXXXX Occasionally some day i get some congestion in chest and after burping it gets relaxed.What is the cause of periodic fever?

I have gone through your query and understand your concerns. I would need to ask you what treatment you have received for typhoid and for how long. Typhoid needs prolonged treatment with 14 days or more or antibiotics like cephalosporins or ofloxacin. Unless that is done, typhoid relapse is quite common. That is probably the reason that you are getting the fever from time to time.

From 31st may i am(7 days now) taking Cefpodoxime tablets 200 mg at 8.00am ,8 pm,total 20 tablet(10 day) course.
Apart from this ,doctor prescribed 15 day course of SORBIDOL 300(Ursodeoxycholic acid tablets) twice day after meal,,,,,,2 teaspoonful SORBILINE Syrup thrice daily,,and BECOSULES Z TABLET after lunch.
At 3 consecutive saturday i got fever of 100.3 F(approx),,,,,22nd may,29th may,4th XXXXXXX so i got worried.My antibiotics course have started from 31st may.

You need to continue Cefpodoxime and the fever can occur for some more days. You need not panic about it. However, I find the other mentioned medicines to be without significance. You may stop or continue them at your own will.

I have checked the urine report and it does not indicate any urinary tract infection.

Other medicines like Sorbidol and Sorbilin that my doctor prescribed due to 1.24 mg/dl Total Bilirubin(what he interpreted to be Jaundice).


Sir, is 10 days cefpodoxime course ok? or Do I need to continue upto 15 days?

14 day course preferable.

I would prefer a 14 day course of Cefpodoxime. Sorbidol or Sorbilin would not help in bringing down your bilirubin level. It will come down on its own. Unless associated with obstructive jaundice Ursodeoxycholic acid has no role to play.

Dear sir,I am once again back at your door.
After completing 14 days course of antibiotic Cefpodoxime for typhoid,I had my blood test done again,which I have uploaded here.
The outcome was negative for Typhoid, improved Total Bilirubin(earlier 1.24mg/dl) but increased SGOT and SGPT levels.
Following are some of my queries:-

1. Doctor has prescribed me 15 days course of URSOKEM-300(Ursodeoxycholic Acid Tablets) twice daily and after that once daily for one more month, due to high level of SGOT,SGPT. Two days ago I had completed 15 days course of SORBIDOL,which I found to be of same composition.I asked my doctor about the same,but he told me there is a difference between the two.
Also Bevon Capsules once daily(15 days).
Are these medications correct?

2.Which foods shall I avoid? Any special food to help recovery?

3.Can i consume sugary supplements like Jaggery,PalmCandy and Honey?

4.Hot meals would be bacteria free,but can I consume lukewarm foods, considering about the liver?

5.Can moderate amount of Ginger and Chilly be given in my food?

6.Can I take TRIFOLA in the morning as a laxative?

Thanks for writing back. Let me try to answer your questions one by one:
1. Sorbidol contains the same ingredients (except for the difference in the marketing company).
The medicines are proper.
2. Avoid oily and rich food. No specific food to help you recover fast. Recovery will occur on its own.
3. You can consume jaggery and the mentioned items.
4. Hot food would not be bacteria free. Bacteria would be everywhere. Stop bothering about them. Take lukewarm foods if they feel pleasant to eat. They are not going to benefit the liver in any way.
5. Ginger: yes, chilli: no.
6. Trifola can be taken without worries.
7. Take steam inhalation multiple times a day. That will help in clearing the nose block.

Are these levels of SGOT and SGPT ,a matter of Concern?
Does it indicate a Liver Disease?
Can Typhoid be a reason behind such elevation? I haven't consumed Alcohol ever or any addictive materials.
Suggest me whether I need any further treatment.

I am fond of Chicken and usually have it twice or thrice a week,but now I am planning to reduce its intake significantly.

The raised SGOT and SGPT are indicative of liver cell injury which could be due to typhoid or infective hepatitis. It will resolve once the underlying issue resolves. Your treatment is proceeding in the right lines and you need to be patient.

Protein digestion involves the liver. Chicken contains protein. So for the time being it might be a good idea to give the liver some rest so that it could heal well.
