Suggest Treatment For Anxiety And Phobia

Question: Hi doctor.

Im 58 years old have 5 child's, and the older one is 40 years old, I have been suffering fear from airplanes, elevator, and being in a car or tight place for 25 years. Am a house wife. I don't afraid from the dark, but I have fear from death.

This problem started when I was traveling and the plane was shaking hard and since then I was at fear. I was in house for 10 years not able to go out. Then I slowly been forced to be driven in car to go short location like mom and sister houses. My husband he is the one who buy me cloths becouse I can't go to far locations.

Then we been located to different house. Still had these problem but it's just less better. But still have fear. I don't have friends, just few that I phone call them only.

My husband also have this issue but in different way, he is one who need to close the ac, light switch, watering garden so many times, do very very routine in and out the house. We don't have XXXXXXX on life, it's just a routine. So I do not expect help from him, as he also not working becouse this issue for 20 years. He also not having XXXXXXX so it effect more to my setuation.

Me children (30yr, 34yr, 37yr, 37yr, 40 yr) trying to avoid me because they are boared of our routine life. My two dughters actually went far of me, and now have only two sons who are planning to be also in distance, I have only the youngest who is triengaged to help but still I make him suffer. My young sun been diagnosed with biopolar disorder, as well my older son have depression.

Few month back I did problem that I chocked with food once, and since then have extreme fear of eating food, do I had just few crashed snack, now I also developed issue with not feeling hungry as I lost 25 kg in 6 month. I did full checkup after sons forced me, I had hemoglobin very low, extreme hard beat from fear of doctors, less vitamin D.

They give me vitamin D injection coz I can't swollow medicine, and hemoglobin syurap.

I been forced to go psychristic witch he gave me cipralex 10mg and Lexotanil half pill morning and night.

So I started to crash them and consume with juice. I had it one and felt extreme bairn in all my body. Then went to general doctor who gave me like a midicine that I put in mouth and to cot the tummy not to feel burn. No use again.

When I had - Busbar buspirone hydrochloride and Anafranil SR 75 I didn't have this problem.

I don't know what to do, please help

Important aspects of your query are:

•     Good response to Buspar (buspirone hydrochloride) and Anafranil SR 75

From your description it appear that you are suffering from agoraphobia which is a type of anxiety disorder. In chronic untreated cases these is risk of developing depression like in your case.

This problem occur due to neurotransmitters imbalance in brain most commonly involving serotonin neurotransmitter. You need treatment with psychiatrist regularly.

In drug treatment two types of medicine used. For immediate relief of anxiety benzodiazepines group of medicines are used and for long term relief most commonly TCA (like Anafranil) or SSRI (cipralex) are used. TCA or SSRI takes around 3-4 weeks to show its effect. Non drug treatment like cognitive behaviour therapy (form of psychotherapy) also takes weeks to show its effect. Buspar is also good for anxiety.

Buspar and TCA (anafranil) is good combination to treat your condition, specially in case of past good response to treatment. You can request your psychiatrist to start them again. Same time you need to monitor your food intake and gain weight.
