Hello,
Thanks you for choosing HealthcareMagic for your query.
Have gone through your details and i appreciate your concern.
As all your baseline investigations are normal the only possibility left to be ruled out is hypothyroidism.
Hypothyroidism can also cause edema so i would suggest you to go for thyroid profile test.In case it comes normal then your ankle swelling is due to calcium channel blocker you are on.One of the commonest and use limiting side effect of Nicardia retard (Nifedipine) is ankle edema.

The prescription given by skin specialist has nothing to do with ankle edema.

Please attach a photo of your ankle (both) in follow up and please mention why are you on Renosave (nephroprotective) and Febuxostat( used in increased uric acid cases).

Suggestion-
Initially go for thyroid function test.
If it turns out normal stop Nicardia retard and continue with other anti hypertensives you are on.
Nitrates can be used as they have minimal side effects.

1. Urea : 29, creatine : 1.6, RBS:74, TSH:3.83, CRP:13.1, Total cholesterol:166 LDL:107,HDL 39, Triglycerides:99 VLDL:20

LDL:2.75, Alkaline phosphatase : 132 (as per range 30-120 U/L) GAMMA Glutamyl Transfernse GGT :108 ( As per 0-55 U/L)

haemoglobin :12.6 24hrs urinary protein :80.52 (as per range <150 mg24hrs) and 24hrs urine creatinine 813.39 (600-2500)

Need your advise on the below 3.

1. my father has concern on the increase of prolomet Xl from 50mg to 100mg. Doctor already advised to use 100mg but still
he prefers to use 50mg. can you use 100mg if yes will he have any side effects with that increase.

2. Nephrologisit advised to use ISOLAZINE (20/30.75) For BP instead of NICARDIA RETARD 20MG TABLET.
In this case also he is very much concern if he uses it may cause some other issue as at present he is sufering with dry
skin issue.

3. FEBUTAZ 40mg, NEFROSAVE can we use these if yes will he have any side effects for the same.

Hi again,
100 mg metoprolol per day is optimal dose of beta blockers. In fact all the cardiac patients are usually maintained at this dose.So as per your query answer is no there is no harm in taking metoprolol at a dose of 100 mg per day. In fact it offers you a extra cardiac protection also.

Nifedipine use is now contraindicated in elderly as it causes reflex tachycardia hence increases the chances of sudden cardiac death due to myocardial infarction. Isolazine is a much better and safer option as compared to nifedipine.
Isolazine has nothing to do with skin drying.

Role of febutaz is to decrease serum uric acid levels, You have not mentioned uric acid levels of your father in case they are normal don't give this drug.

Renosave is a kidney protector and can be given.
