Suggest Treatment For Anal Fissure

Question: I have been suffering with an Anal Fissure since the end of January of this year. The doctor started me on lidocaine which burned so bad I just couldn't bear it. Since then I have been on several other creams to help heal this literal pain in the ass. More pain than I have ever endured before. I had my first fissure when I was 27 years old. This one is just relentless. Nothing has eased the pain. I was rolling around in pain for hours in spasm. I was put on tramadol for pain and then a muscle relaxer to help the spasms and the pain. I would take a muscle relaxer 1 to 2 times daily and 1 tramadol with 3 advil once a day. Finally I was given hydrocordisone acetate suppositories. I have finally gotten some relief after taking these for two weeks. I am out of state (so my ins. is out of network and won't cover the surgery that I was told that I needed to have. My question is this. I still have some pain but much less...thank the Lord. After I stop using the suppositories will the pain come back? I am due to go back home in two weeks and have enough suppositories to last me till then. Do I continue this medication even after being on it already for two weeks? But most importantly is it just covering up the fact that I will in fact to have the surgery in the end? I would appreciate any feedback on this. Thank you kindly.

Hi and welcome to HealthcareMagic.
In my experience,and evidence support this, the best management of fissures is procedure called lateral sphincterotomy which is permanent solution. Everything else is just used for symptom relief and prednisone cream or suppository is an example. it is not advisable to use it over a longer period since it may cause perianal skin irritation.So I suggest to use it only when pain is significant.
