Question: Good morning

I want doctor chandrakanth
I was discussing about my matter. You ask me about the wound , it was looking good few days back thts y i didt answer you. The wound is getting little bad now. I can see small amound of pus in that. So its showing taking very ling time to heal. Its almost 1 month after surgery. Whats your call on this

Hi.
Thanks for your review query.
Noted that the wound was looking good a few days ago.
Now can see a small amount of pus, it is almost 1 month after surgery.

My thoughts:
Ultrasonography of the abdominal wall and abdomen as it can show whether there is collection of pus in the wall or inside the abdomen and if necessary to get a confirmed diagnosis by CT scan.
This may be a collection or just a stitch abscess.
Try to send the sample of the discharge for gram staining, culture and sensitivity and telling the laboratory about the past findings of actinomyces so that they can guide the best. All this before the antibiotic is started to get proper results.
After the sample is sent for lab you can be started on appropriate antibiotics.
The Ultrasonography report will help to see whether there is a collection or just a stitch abscess.
If this is just a stitch abscess, small exploration and cleaning and removal of the stitch will cure your the problem.
If there is more than that, you may need a bigger exploration.
It is possible that a proper antibiotic cover may cure you problems and the culture and sensitivity report if positive will further help for proper antibiotic coverage.

My surgery was done on march 10 2016. I consult doctor on 27 th march due to infection in incision.on the same daythey took culture of pus and the result came NEGATIVE. BUT THE DOCTOR TOLD ME(NOT MY SURGON HE WAS ON LEAVE)
THE ACTINO MYCES INFECTION HARD TO FIND.ON 27 TH IT SELF MY ULtRA SCAN DONE..REPORTS ARE BELOW. AFTER ONE WEEK WHEN MY SURGON CAME AND I MEET HIM,he told me no need to continues medicine as far as i have sleep ,motion and hunger iam okay.that was the time i first mailed you.
any how i was taking 300 mg in morning clindamycin 300 mg in evening after his deniel also.but i continue to go with him every two days for dressing my wound.till april 8 i go and he told i dont need to go after. day before yesterday i took the dressing out.its look like heeled but stil small amound of pus is there inside.so iam thinking if it was normal infection its already got time to heel?
even if its small pus its showing out.but again on 27 th the culture showed negative..which makes me confusion .kindly observing my ultra scan and wound nature, time of heeling character of my wound i need your great opinion..thank you.

Thanks for your appreciation and detailed history. The ultrasound is old and shows mild inflammation and fluid without any loculation.
It is true that a good clinical recovery is shown by good appetite, sleep, bowel movements without any symptoms or signs in general or related to abdomen.
As said this may be a small subcutaneous collection.
Please follow the advise and you will be fine.

Sir, physicaly i dont have any other problem. Only thing is this very minor wound look like almost heeled, but can see pus thr.
Since i dont have any other problem should i need to take ultra sound?

Or i will take tablets

Hi.
We have already discussed the need for an ultrasound of the abdominal wall as well as the abdomen basically to rule out any larger collection and to see if there is just a small stitch abscess or so.
The larger collection can have only a small opening in the skin in the beginning. Seemingly minor wound can not tell us the extent of the collection underneath.

Nice to know that there are no other problems.
Tablets may need to be taken.
