Question: For months I have felt heavy in the chest, sore stomach and generally not well, I have had about 8 ECG's and a exercise ECG which all have come back fine, my doctor tested me for every blood test just about and it came back that I had helicobacter pylori, I have since been treated for this and am still having stomach ache, heavy chest and feel like I am not breathing properly, lump in the throat and generally unwell, I have also just been diagnosed with gallstones to, you have any ideas what may be causing these issues

Brief Answer:
Could be H.pylori infection relapse or gall stones.

Detailed Answer:
Hi,

Welcome to health care magic,

Your symptoms like stomach ache, heavy chest and difficulty in breathing could be due to acute gastritis as a result of helicobacter pylori infection here.

It does not seem to be related to heart as your several ECG and TMT came normal as stated by you.

It would be useful information for me if you could provide that which antibiotics combination therapy you have been treated for H.Pylori infection.

Have you undergone investigations like routine blood, antibody testing for h.pylori, stool antigen test or gastric biopsy or gastroscopy after the full course treatment with antibiotics given for H.pylori to make sure further that you were free from h.pylori and treated radically.

Your gall stones could also be the contributory factor for stomach ache and epigastric pain and feeling unwell.

You can go for liver function tests and ultrasound for hepatobillary to rule out any complication related to stones in the gall bladder.

You must consult gastroenerologist for your h.pylori, gall stones and throat lump as stated by you.

You can ask me if there is any doubt further.

Thanks and Regards,

Brief Answer:
Triple drug regimen has failed or it could be resistant strain of h.pylori.

Detailed Answer:
Hi,

Thanks for the follow-up query,

So,you were taking triple drug therapy for h.pylori infection. If you still having no improvement then, either the drug regimen has failed or you might have developed resistant strain of this h.pylori.

You will have to undergo blood culture and sensitivity to detect the sensitivity of the drugs to this h.pylori infection with other investigations as i mentioned in previous answer.

You must consult gastroenterologist for further treatment as i said earlier and definitely not the family physician.

If there is failure of triple drug regimen or resistant strain of h.pylori then one can try alternative triple therapy or quadruple therapy.

This alternative triple regimens I am giving below is not to confuse you but the aim is to clear all the possible regimens what you can try but only after your Gastro-physician's consultation.

1) alternative triple drug therapy

(i) omeprazole 40 mg daily+ amoxycillin 500 mg thrice a day + metronidazole400 mg thrice a day for 7 days full course.

(ii) ranitidine 300 mg daily+ amoxycillin 750 mg thrice a day + metronidazole 500 mg thrice a day for 12 days course.

(iii) colloidal bismuth subcitrate 120 mg 4 times daily + tetracycline 500 mg 4 times daily + metronidazole 400 mg 4 times daily for 2 weeks.

(2) quadruple therapy

pantoprazole/ rabeprazole 40 mg twice daily+ colloidal bismuth subcitrate 120 mg 4 times daily+ tetracycline 500 mg 4 times daily+ metronidazole 500 mg 4 times daily for 7 days full course.

You can discuss all the above alternative regimens to the gastro-physician and he can prescribe you the best among all regimens according to your medical history and suitability.

Hope this will clear all your doubts now.

Thanks and Regards,

Dr.sandipkabra
