Question: dent 57 yrs old..very active. Food comes back up at night with or with out acid reflux. Often choking and disturbing sleep. Had bad accident 5 years ago broke neck and was full quadrpaligic however worked hard for years regained 100 % of extremidies. residual numbness hands chest et but certainly not complaining

Thanks for the query.
I have read your query and understand your problem.
You seem to be suffering from GERD which is generally caused by weakening of junction between esophagus and stomach known as ( XXXXXXX ...lower esophageal sphincter ) ,
Weak junction results in flowing of stomach contents back in esophagus and throat.
Sometimes a condition called hiatus hernia can also lead to it.
The most preferred treatment is change of lifestyle,diet and medications.However some people may require surgery.
What to do....
Eat small portions
Eat light and 3 hours before dinner
Raise your headend of bed by 5-6 inches by putting blocks below
Avoid smoking
Reduce your weight
Avoid citrus fruits,pepper,tomatoes etc which irritate your damaged esophageal lining
Avoid foods which weaken XXXXXXX like peppermint,alcohol,coffee,chocolate,fatty and fried foods etc.
May take a proton pump inhibitor like PANTOPROZOLE 40 mg a day in morning on empty stomach.

If you don't find relief with these measures,then you would require a comprehensive evaluation and further management.
