Suggest Treatment For Acid Peptic Disease

Question: I suffer from acid reflux. I am 37 years old and married. The problems started some three months back. I was fasting for religious purposes. And felt burning sensation when i broke fast. Went to doctor who presecibed me NEXPRO FAST 40 for 10 days. The problemn went away. But every now and then, that is 7 to 10 days I get this problem,. NEXPRO FAST 40 bring relief. Now I am expereienicng a piercing pain under my right rib cage possibly in my liver, since the last 3-4 days. I dont have yellow eyes or jaundice...
What is a permenent remedy?

Hi, I am Dr Anshul Varshney , I have read your query in detail , I understand your concern
You are suffering from Acid Peptic Disease.
Frequently, there is association of h.pylori infection with acid peptic disease, which requires elimination to control the acid peptic disease.
I would advise you following:
1. Ask your doctor for h.pylori infection test, if it is positive you should be given treatment for h.pylori infection as it prevents healing.

2. You need to add Domperidone to prevent reflux. You can take a combination of Pantoprazole and Domperidone.

3. Take soft, bland diet.

4. Take frequent light meals instead of 3 major meals.

5. Always walk after you take your meal.

6. If you take alcohol and smoke, you should refrain yourself from them.

If h.pylori infection is not there, i personally advise 2 months of treatment with the combination i have mentioned along with life style changes that i have enlisted.
My patients gets very good improvement in 2 months.

I visited a doctor with your advice. He has agreed with your diagnosis. I showed him the computer printout.
But he ruled out tests, as unnecessary. He has prescribed the following drugs.

1. NOVAMOX . THREE TIMES FOR 5 DAYS.
2. ORNI 500 MG - TWICE DAILY FOR 5 DAYS.
3. OMETAB - SINGLE DOSE FOR 20 DAYS
4. ACINOL-O - FOUR TIMES DAILY AFTER MEALS.
5. COLINOL - ONLY IF THERE IS PAIN
6. ZYMINO - TWO SPOONS TWICE DAILY FOR PROPHYLACTIC USE

I seek your suggestion with regard to this prescription.

Yes prescription is fine.

Detailed Answer:
Dear Friend.
It's all right if he has omitted the test because the test mostly comes positive.
As far as treatment regimen is considered, there are many treatment regimens. Every one uses according to there experience. The one advise by your doctor is also fine.
However, I personally prefer to use regimen that includes Clarithromycin.
For patient's convenience, I prefer to use h.pylori treatment kits widely available in the market. Most commonly available as Pantocid HP kit. It contains 6 tablets. 3 tablets to be taken in the morning and 3 tablets to be taken in night.
I prescribe this for at least 10-14 days for complete eradication.
So, you can continue with the regimen as advised by your respected doctor, when you go after 5 days to him, share my opinion.
