Hello again.

Thanks for getting back to me with the answers.

Based on your description and history, it appears to be a muscle sprain. Continue the symptomatic relief meds for the time being i.e the motrin and muscle relaxant. However, musculoskeletal pain should respond excellently to ibuprofen and muscle relaxants such as cyclobenzaprine or tizanidine. Though you took the muscle relaxants just now, it usually takes 40 minutes up to an hour for oral medication to show there effect. PPIs like pantoprazole can be advised for 2 weeks for the bloating and GERD symptoms associated with the pain. Long standing constipation can also cause cramps and abdominal pain including lower back pain.

I would advise you to get the following investigations done after consulting your doctor in case the pain persists or worsens, to determine the possible cause.

Urine Routine exam plus culture and sensitivity testing
X ray lumbosacral spine AP and Lateral view
Ultrasound abdomen and pelvis
Plain X ray erect abdomen if there is abdominal tenderness, guarding or rigidity (acute abdomen)
Stool routine exam plus ova and parasites

You may discuss these options with your doctor and he/she can guide you further.

Wishing you best of health

Thanks

Dr. M.S. Khalil
