Suggest Treatment For T-cell Lymphoma

Question: Hello! You may want to refer to my previous questions to understand my situation. I want to bring you up to date on where I am now. I have been seeing a hematologist (female) in the city. She looked at me and heard my symptoms and told me that I had t-cell lymphoma. This is without any labs or biopsies. She said I had all the symptoms and was sure that was what I have. That was 3 weeks ago. She had me go across town to a dermatologist to have a skin biopsy taken under my left armpit where the swelling and redness were the worst. He took the biopsy but said he did not believe I had any lyphoma but sent the biopsy to the best lab in the country in XXXXXXX and one in XXXXXXX I went back today and both biopsies were benign. He has put me on a cortisone cream for the red areas under my armpits. It has helped some, but still red. He believes my whole problem is dermatological and that I have been exposed to some toxin, antigen, irritant that has set off my whole lymph node system. He wants me to come back mwf next week to do a series of tests on my back for allergens. He will use the most well known allergy things and see which one or ones react negatively. I know this seems rather strange, but I guess I will try to do it. Mean while the hematologist that sent me to him took a lot of blood samples 3 weeks ago from me and was to have them tested. Unfortunately, my insurance carrier put a hold on them and took 12 days to make determination if they were going to cover the expense. I was then told that I would have to come back and they will take more blood samples and do the testing. I was there today for more blood samples taken. The ones that they were able to do turned out good. All my vitals looked good. Of course, the ones that matter are the t-cell labs that they re took today and I wont know the results for another week. I would like to give you all my symptoms and have you tell me what you think is wrong with me if you would. I am very sick. I have swollen red lymph nodes under both armpits. The skin around them about 10 inches in diameter is red and hot. I also have a lump on the base of my neck and collarbone, but no redness there. I run a fever every afternoon and evening. Without ibuprofen it gets up to about 101.5. I have extremely dry mouth and eyes and my tongue burns when I eat or drink. I ache all over like I have the flu, but ibuprofern helps this . The headaches are not too bad, but ibuprofen controls them every 3 hours. The lymph nodes under both armpits get very tender and sore if I miss taking ibuprofen every 3 hours. I have told you before that dexamethasone clears it all up in hours, but I cant take it or prednisone while they are still testing my blood. What in your professional opinion is wrong with me. I am very frustrated that no one except the dermatologist seems to have an idea. The hematologist may find the t-cell lymphoma that she claims I have, but no previous tests have showed it. thank you.

Brief Answer:
You may have skin or lymph node pathology.

Detailed Answer:
Hi, dear
I am Dr. Hardik Sanghvi answering your query.
I have gone through your question. I can understand your concern.
You have enlarged lymph node and some skin problem with fever and pain.
You may have reactive lymph node enlargement due to some skin problem.

Second possibility is lymph node pathology. You may have tuberculosis or lymphoma and it secondarily affect your skin.

You should go for biopsy of lymph node. Then go for histopathological examination and IHC marker study.

It will clear your all doubts. If you don't have primary lymph node pathology then consult dermatologist. No need to consult haematologist.

If you have lymphoma or other lymph node pathology then provide me the report. I will help you.

Till that time you can take symptomatic treatment with ibuprofen for pain relief.

Hope I have answered your question, if you have doubt then I will be happy to answer. Thanks for using health care magic. Wish you a very good health.

I had surgery to remove 2 lymph nodes on left underarm a couple months ago. The pathology showed no lymphoma at all. Pt scan also showed no lymphoma as well as bone marrow biopsy. However this one hematologist is convinced I have t cell lymphoma without even testing. I guess the blood I gave today will either disprove or confirm her suspicions. thanks.

Brief Answer:
You need IHC study on lymph node biopsy.

Detailed Answer:
Hi, dear.
I have gone through your question. I can understand your concern.
You should go for IHC (immuno histochemistry) marker study on your lymph node biopsy. It will clear all doubts. Blood examination will not be helpful in all cases. Go for IHC marker study.

Hope I have answered your question, if you have doubt then I will be happy to answer. Thanks for using health care magic. Wish you a very good health.
