Suggest Treatment For High Post-prandial Hyperglycemia

Question: yesterday i wanted to see how and if my pancreas was doing.
i went to an event and ate 2 pc pizza and drank what i was told was tea but ended up was some kind of juice. i felt a little wierd when i got home the sugar was 333 took 10 units novolog and 15 units lantus long term
got it down to 77 blood sugar in about 2 hours. 24 hours after i feel like i took it to the edge and was a really dumb thing to put the body through . i am type 2 and newly learning how to be sane with the food and have had it under control . recently lost 50 pounds , but this situation was scary. i guess the question is how long should it take for the body to recover from this stress event? i have been trying to keep bs from 80- to around 100. thanks- granny workin hard. NovoLog (insulin aspart [rDNA origin] injection) is a fast-acting mealtime insulin.Its onset of action is <0.25 hours ;peak action 0.5–1.5 hours;effective duration of action 3–4 hours.
Lantus (Insulin Glargine ) is a long acting form of insulin. Its onset of action is 1–4 hours;effective duration of action up to 24 hours. Glargine has minimal peak activity i.e it is a basal insulin.
After a binge eating you can take an additional dose of fast-acting insulin like NovoLog.
However basal insulin like Lantus should be taken once a day only not with every meal.
Target goal of blood sugar is Fasting 100 mg% and PP 140 mg%.
In general, the target A1C should be <7% with a more stringent target for some patients. A higher A1C goal may be appropriate for the very young .
You are 74 years.
Targets for Glycemic Control in the Elderly is a little less stringent.
Older diabetic patient in good health :
Fasting blood glucose between 90 and 126 mg/dl. HbA1c between 6.5% and 7.5%
Older diabetic patient Frail:
Fasting blood glucose between 126 mg and 160 mg/dl.
HbA1c between 7.5% and 8.5%

YES SIR IT CLEARED UP HOW I AM TO USE MY INSULIN.I WOULD HAVE LIKED TO KNOW ABOUT THE SORT OF CASCADE EFFECT FROM WHAT FOR ME WAS SUDDEN AND UNEXPECTED QUITE HIGH INSULIN. (333) COULD YOU GIVE ME A REFERENCE FOR THAT???

Hi,

Thank you for your query. I can understand your concerns.

Extra binge of meal can give rise to high post-prandial hyperglycemia. In any such event in future, I would start one tablet of voglibose (0.3 mg) before a large unexpected meal. Your treating doctor will know about this drug.

Acute stress sometimes also can give rise to temporary high blood sugar-usually controlled with extra dose of regular insulin.

Reference can be taken from any standard textbook of medicine but may not of much use to lay /non medical person
