Suggest Treatment For GERD In Child

Question: I have a 3.5 yr old son. He has had child asthma and was on budicot + levolin respules twise a day. Now its only levolin once a day and doc has asked to continue for 3 days more and then discontinue. I have time again asked her why does he complain of stomach ache when running and she said its due to this breathing problem. However lately i see it more often on running. I think issue might be his daily routine where after his meal he hits bed, within 15 mins. Also he has his milk in bed. So when he gets up in the evening and runs around he gets stomach pain and throws up. Even at times after dinner i see he coughs and throws up. Is this GERD...???

Dear Mr Bhatia

Based on description possibility of GERD needs to be considered. Pain in abdomen is unlikely to be due to asthma.
The most commonest cause for pain in abdomen in toddler are
1. Constipation
2. Worm infestation
3. Gastritis and associated GERD

It would be very helpful if you could provide me with the following additional information

1. Is he passing stools regularly?
2. Does he strain excessively while passing stools?
3. Where is the exact location of pain ?
4. Does he have any urinary complaints?

Yes he is passing stools regularly. He does make a face like he is pushin hard, if thats straining hard dunno. He says it pains in the stomach...at times points to left side or there are times when he says in center. Its mostly after running. I dont see him having any urination problem, but yes at times he tries to say that it there is a burning senstion. I ask becoz today aroun 4.30 pm he got up from sleep (sleeps right after having his lunch) had some milk from his bottle and went down to play. He came up and burped and then puked. Puke as in all came out. Then he had some soup and after almost 1.5 hours had dinner (1 roti). Came to the bedroom coughed and puked again. This puke after running has happened earlier also and he complains about pain often. Pain is definately not very bad because he doesnt show too much of discomfort. I think its slight pain or uneasiness. Please advise.

Likely to be due to reflux

Playing immediately after drinking milk could ininduce vomiting due to reflux which is quite common on physical exertion. Pain in abdomen is also likely to be due to associated worm infestation and constipation.

I would suggest you the following

1. Avoid playing immediately after drinking milk or fluids

2. Ensure sufficient intake of water : 1 cup of water 1 hour before meals and 1 cup of water 1 hour after every meals will ensure adequate intake of water

3. Deworming with antihelmenthic

4. Avoid food stuff containing maida (refined wheat flour ) like bread , biscuits. They are one of the cause for pain in abdomen

5. Ensure adequate intake of fiber in diet : 2 servings of vegetable salad and citrus fruit will be helpful

6. Antacid like Lansoprozole dispersible kid tablet taken once a day for a week will be helpful

Antispasmodic syrup will be helpful

I hope there is no associated fever and vomiting. You can give him antispasmodic syrup like Colimex syrup.
If the pain still persist till morning then do consult Pediatrician for further clinical assessment and to rule out infective focus in abdominal cavity.
