Brief Answer:
Testosterone

Detailed Answer:
I am sorry to note your ED problem.

I see your FSH and LH are low but it is more important to interpret these in light of the blood testosterone levels.

So it would be necessary to do a blood test for Serum Testosterone in the morning before 9 am or so. Later in the day, testosterone levels normally decline and you would read a falsely lower value.

The ideal test would be to do a Serum Free Testosterone by Equilibrium Dialysis method. So if you have a local laboratory which can do so, it would be perfect. However, as this test is not easily available, the following tests are good alternatives:

Serum Total Testosterone
SHBG
Albumin

These will allow us to indirectly calculate the estimate Free Testosterone, which gives a fair idea of the actual free testosterone concentrations in the blood.

When I se someone like you in my practice, I also order the following tests in addition to a thorough physical examination:

CBC
Electrolytes
Liver function tests
Kidney function tests
TSH
Free T4
Prolactin
HbA1c

I have taken note of the other copings such as the constipation and the cyst like split in your testicle. this will require an in-person assessment. So I suggest you see a Urologist for this
