Hello thanks for asking from Health Care Magic

I have read your query and I can understand your concern. You are a young male with history of depression and on treatment of depressive symptoms. Most of individuals taking treatment of depression improve with medicines but some individuals fail to respond. Some nutritional deficiency or hormonal deficiency might be the underlying reason for that or poor response could be due to resistant depression.

Low vitamin B12 levels are one of common finding in poor response. I would advise you to get your B12 levels checked and start taking supplements of Vitamin B12.

Low thyroid levels are second possibility. Get your thyroid function tests done to rule out this.

If these investigations come normal and you are also showing poor response then we can keep resistant depression as possibility. You haven't mentioned the symptoms and history of your depression, if possible please mention in follow up question. Also provide details about your treatment.

Usually medicines like SSRIs or SNRI drugs are used for depression. If some individual fails to respond then augmentation of drugs with Lithium or by some other anti-depressant of different class (mirtazapine, bupropion) can be done. These augmentation drugs help to improve symptoms and hasten recovery. Mirtazapine and bupropion are also free of serious side effects. Consult your psychiatrist for prescription of these drugs. I will request you to please provide details of medicines you are taking so that I can provide the management in details. But depression in most individuals respond to these augmentation drugs.

I would advise you to attend Cognitive Therapy sessions by a psychologist. Depression at your age respond to COGNITIVE THERAPY. In cognitive therapy abnormal faulty thought process and cognitive distortions are found and then in phased manner correction of cognitive distortions is done.

Thanks, hope this helps you, Ask again for more doubts.

Brief Answer:
Anxiety and depression

Detailed Answer:
Hello again and thanks for asking follow up

The symptoms started with intense fear and anxiety which progressed to depressive symptoms. The symptoms like fast beating heart are due to anxiety. Poor performance in studies and anger outburst are your other symptoms.

From your history it appears that anxiety of performance aggravated the depressive symptoms. Most of antidepressants also have anti anxiety action. I would advise you to consult a psychologist for relaxation methods like progressive muscle relaxation exercises and breathing exercise for relief of anxiety.

As I have mentioned already cognitive behavior therapy have excellent results in such symptoms. A good psychologist can help you.

For symptomatic relief: In states of anxiety medicine like low dose benzodiazepines as Etizolam, or propranolol can be used. These drugs help to reduce intensity of anxiety symptoms like palpitations.

Hope this helps you; spend your time in exercises and in listening good music. Consult a psychiatrist only if symptoms persist. Start taking vitamin B12 supplements.

Thanks, ask again for more queries

Thanks for asking again

Epitril is Clonazepam and is a benzodiazepine drug. You have been given the tab twice a day. Benzodiazepines are known to cause dependence so I would advise you to not to take this medicine continuously for more than 4 weeks, it's not advisable as per guidelines. Consult your psychiatrist for change in prescription.

Betacap is propranolol only and the drug will help to decrease anxiety without risk of dependence. As per my opinion you are on good dose of this drug. Continue to. Take the drug as per prescription.

I apologise but I didn't find constituents of Ferizac, as it’s a brand name. As per my opinion there is need to add some antianxiety drug like SSRIs to your prescription Fluoxetine, Paroxetine, Sertraline etc can help to decrease depression and anxiety. The medicines help to correct biochemistry of depression. They start showing effects in about 4-6 weeks and result in improvement. Consult a psychiatrist for prescription of these drugs.

The poor performance in studies is due to anxiety and depression only, with improvement of symptoms your performance will improve. No need to fear for that.

Thanks, Hope this helps you, if you have more doubts you can ask again.
