Suggest Tests To Diagnose H.pylori Infection

default
Posted on Fri, 17 Apr 2015
Question: I had an ovary removed to to torsion and endometriosis last year. A few months later I had an appendectomy for adhesions. Since the appendectomy I have had severe hemorrhoids, horrid reflux, and a constant gurgling stomach. I am unable to have a colonoscopy for a year due to the recent surgeries according to my GI doctor. I am very upset and want to have testing done for H pylori, and other inflammatory conditions of the GI tract. Is it at all possible to have these tests through blood work or stool samples instead of invasive testing? I'm very worn out from all the medical things I have been through. Also, is any of this common after these surgeries? I find it hard to believe I just developed three new medical problems in one year. I'm 35 and female, no prior medical issues.

Hello,

Welcome to Healthcare Magic.

I read carefully your query and understand your concern.I am sorry that you are going through all these health problems.

There are some tests that you can do for example :
-H.Pylori stool test ,that can determine the presence of antibodies against H.pylori in stool.
-Fecal occult blood test (tests for hidden blood in stool)
For the diagnosis of Inflammatory bowel disease (Crohn disease or Ulcerative colitis) laboratory tests are not specific. But some blood tests can give additional information like that can be related to inflammatory bowel disease:
-Complete blood count to exclude anemia ,which is common in this condition. This might be due to chronic blood loss or anemia due to malabsorption of iron ,folate and Vit B12.

-A high level of white blood cells (WBC) is common in patients with active inflammatory disease
-Inflammation markers like ESR and CRP.High levels indicate the presence of an inflammatory disease.
-Fecal calprotectin levels

Since these conditions are autoimmune disease ,there are some serologic studies like testing for specific antibodies that are found in these conditions like:
-pANCA (Perinuclear antineutrophil cytoplasmic antibodies )
-ASCA (anti-Saccharomyces cerevisiae antibodies )

All these tests can indicate the presence of inflammatory bowel disease but are not specific .The diagnosis is confirmed with colonoscopy.

Hope it helps.
Let me know if you have any other question.I will be happy to help.

Ok, but why is this happening all at once? I feel terrible most of the time because of all this new medical stuff. Is it the appendectomy or ovary surgery that set the stomach issues off? I eat healthy, exercise, and dont smoke or drink.

Hello again.

Surgeries represent a stressful event for the body, when a lot of stress hormones like cortisol and epinephrine are released in bloodstream. These hormones associated with postoperative antibiotics ,painkillers can induce stomach conditions like gastritis, peptic ulcer, acid reflux.
So it is common to experience stomach and gastrointestinal disturbances after surgeries but usually these symptoms may last up to several weeks to few months.

A severe acid relfux that doesn't respond completely to treatment, need to be evaluated for any underlying cause.

A distant shot might be hiatal hernia. This is a condition where a portion of stomach pushes upward the diaphragm. The diaphragm is a muscular structure that separates abdomen from chest. Your diaphragm normally has a small opening through which esophagus passes on its way to connect to stomach.Due to different causes that increase abdominal pressure (obesity , exercises such as weightlifting, laparoscopy etc) can enlarge these opening causing a small part of stomach to enter in.This causes the stomach acid to move easily in esophagus causing acid reflux.
If your appendectomy was performed through laparoscopy ,which use gas to distend the abdomen , hiatal hernia should be taken in consideration.
The diagnosis is determined with upper endoscopy or an barium X-ray.

You can also do a H.Pylori antigen stool test to exclude H.Pylori infection.

If your symptoms of acid reflux doesn't improve despite the medication ,you may need to do another upper endoscopy.

Regarding hemorrhoids it is very common for them to develop or to get worse following a surgery because constipation ,bed rest, limited physical activity increase the probability of having hemorrhoid.

I can tell for sure why all these problems appeared right now.If the abdominal surgeries caused them or just triggered and make them symptomatic.

Hope it helps.
Let me know if you have other questions.

Kind regards,
Dr.Rovena
