Question: I had a condom break which lead to unprotected sex. I was the insertive partner to a female who was in the end stages of menstration. Prior to vaginal sex, she gave a toothy oral sex that resulted in a bruise/cut ln the head of my penis.

I worry as she is promiscuous around the world (but not africa), though she claims to always make her partners use a condom for vaginal sex. She does not do anal sex or drugs. She does not have partners wear a condom while providing oral sex on men. She claims to be HIV/STD free, but her status in unknown. She hasnt been tested in 4 months and has had sex with 15-30 men.

Should I take PEP, as i am within the 72 hour window?

Hi
Thank you for follow up query.I am Dr Bharatesh D Basti answering your query

Welcome to health care magic. I have gone through the details.

The chances of transmission of HIV by unprotected intercourse with HIV positive person is 1/200 to 1/1000.If condom has slipped it is considered as unprotected intercourse.
As the lady has high risk behavior and the status is unknown,It can be considered as risk for transmission of HIV.
You can get yourself and her tested for HIV/STDs.

You can discuss with your treating doctor and go for PEP to be on safe side .PEP has to be initiated within 72 hours.
You can get back for any clarification.
