Suggest Systematic Treatment For Chronic Pain Around Shoulder

Question: Hi,
This question is for my father, who's 57 years old. It's around a chronic pain he's going through at his right shoulder area (possibly cervical pain) since a long time. It keeps on paining day and night, and whenever you press it/ massage, it 's a reliever. At his early childhood, he used to carry heavy weight, so possibly it may be because of that. However, similar problem is with me as well. Can you please suggest a systematic treatment plan that can give a permanent relief to both of us.

Thank you for posting your query.

I have noted your father's (and yours) symptoms. These are suggestive of pinched nerves in the neck region. These nerves travel to shoulders and arms.

The pinching of nerves could occur either due to his old injury or his habit of carrying heavy weights during his early childhood.

The initial treatment would consist of:

1. Pregabalin capsules- the usual dose is 75 mg taken once daily at night. Depending on the response, the dose can be increased.

2. Vitamin B 12 supplements, such as Methycobal tablets can be used.

3. Physiotherapy with pain relief measures such as IFT, ultrasound, traction, etc may be tried. Later on, neck extension exercises may be started.

The above treatment can be given for one month.

If there is no improvement with the above treatment, an MRI scan of cervical spine may be done.
