Suggest Remedy For Severe Penile Itching

Question: hi, i am suffering from severe itching in penis and anal area.
There were some small boils (not warts) seen near genital area few years back.
Burning sensation after intercourse (even after using condom).
I am having this problem since 7 + years.
I have already taken tests like HivI and II tests and the results were negative.
Seems like some STI.
please advise some tests and medicine.

I have gone through your query and I have noted down your concerns.

I would keep a possibility of Candidal Balanoposthitis.
Candidal infection usually presents with redness and swelling of prepuce, white accumulations between prepuce and glans, circumferential fisuring of prepuce. As urine flows over this inflammed portion of prepuce it can cause burning sensation over the inflammed inner surface of prepuce.
I would like to know the character of vaginal discharge. Is it white, curdy? Or is it thin, yellowish and foul smelling discarge? Do you have a history of extramarital sexual contact?
You say that your wife too has heavy vaginal discharge therefore it is likely that both of you have a candidal infection, which is easily transmitted between husband and wife during sexual intercourse.
Therefore basd on these facts, If I was the treating doctor I would suggest a single dose of an Oral antifungal e.g Fluconazole 150 mg tablet, for both husband and wife.
Topically you may use an antifungal+steroid topical preparation e.g clotrimazole 1% and beclomethasone dipropionate 0.025% (Clotrin-B cream) and your wife can use a clotrimazole vaginal pessary once daily at night for a week.
Avoid getting intimate with your wife for a week. I would also suggest a blood test for fasting blood sugar.

My reply is below.

- It is thin with foul smell sometimes. Often she use Lactacyd wash, clingen suppository and Surfaz - SN ointment. Also I do get itching in both testicles frequently.

Do you have a history of extramarital sexual contact? - Long years back, once i had. Seems the problem started from there itself.

- In the past we had been treated with FAS-3 kit and single dose of Flucanazole once, but still we have this problem. BSF test was taken 6 months before and the results were normal. Also she faces pain sometime while having intercourse initially.

Hi.

Thin, yellow and foul smelling vaginal discharge can be due to chlamydia Or bacterial vaginosis. She has recently been treated with flagyl (metronidazole) which is the treatment of choice for bacterial vaginosis therefore there is no need to retreat her with metronidazole and probably bacterial vaginosis was not the underlying cause because her condition has persisted even after treatment.
Chlamydia can cause foul smelling vaginal discharge along with other symptoms such burning sensation on passing urine, pain during intercourse and pain abdomen. Even though chlamydia is sensitive of azithromycin (present in FAS - 3 kit) but it may not respond to a single dose treatment of azithromycin. Therefore I suggest that both of you be treated with Doxycycline 100 mg twice daily for a week. Abstain from sexual intercourse for a week.
Alternatively, it would be useful for your wife to first get tested for chlamydia and gonorrhea with a Swab for NAAT (Nucleic acid amplification test) prior to commencing treatment. She can consult her doctor regarding the test.
